The statement, "Fibrosis at the left upper lobe with granuloma formation is entertained" is consistent with continuing resolution, with scarring, of an infection, TB or other.
The first step is for your doctor and the radiologist to discuss whether the prominence is real or apparent because of a slightly different technique. If deemed to be real and since the words "more prominent" appear in the report it will then probably be necessary for you to have limited CT scan of this area for more detailed characterization of the lesion.
If the issue is not definitively resolved with the above, it would be wise to seek consultation with a pulmonary specialist and, if you are raising any sputum to have it sent to the lab for direct (smear) examination and culture for bacteria likely to have caused this fibrosis.
That the rest of your lung fields "are clear" is very good news. That the change as described seems to be minimal, that is also good news.
