constant white phlegm, white tongue

Well for the last couple years it seems that i have really bad post nasal drip which causes me to cough up buckets of phlegm every day. i've been to many doctors which have been little help. ( except for catching the pneumonia that this has caused. ) one doctor seems to think i have GERD, i have been taking nexium for the last 3 weeks with no improvment. for the last week i stopped eating dairy, glutton, and my sugar intake is very low, i've heard that it may take longer than that to see results if its a food allergy. anyways it's getting really bad, i'm getting little sleep. I also think it is the cause of my ear infection(s) although the last doctor did think i have one even tho i clearly do. it seems to be at it's worst right when i wake up and right before i go to sleep for some reason. also this all started shortly after i moved from the coast to the mountains (small town to big city) probilly unrelated tho. Until currently I drank almost every weekend, smoked a pack a day, and smoked marijuana pretty heavily, I no longer smoke pot or drink and im down to about 4 smokes a day. any help would be greatly appreciated.

also i very rarley have chest pain but it does come and go and recently i have had no appitite at all.
