sleep apnea results

Hello,  I'm 255 pounds/30 years old.    I recently had a sleep study and I won't bore you with whole report but can you tell me how severe this sounds and would you consider CPAP mandatory?   My main SYMPTOMS have been waking from my sleep with sinus tachycardia and shortness of breath, as well as depersonalization and sleepiness during the day.    Does the follow explain this?    I've been on CPAP a couple weeks and haven't seen much improvement, but I'm having some trouble complying too  (pull my mask off in my sleep),  I have had 3 successful full nights with it on..    It was a split study, and:

"The study started at 10:40 and ended at 02:30.   Total rec. time was 229.8 mins.  with a total sleep time of 95.5 minutes.....sleep latency was 68 mins...etc etc...

Pt. had 37 total events, with 37 arousals,  RDI of 23.2/hr.    0 total obstructive apneas, 0 mixed apneas, 0 central apneas, and 37 partial apneas.   0 events occurred in Stage REM, 37 events were noted in NREM, for total resp. distrubance index of 23.2 events per hour of sleep.

Basal 02 sat 97.1   20 desats dropped by 2.5% with 2 desats below 90%...lowest desat was 89.0%

0 myoclonic events w/ arousal,  with index of 0 events per hour   etc etc etc.

Your daytime sleepiness and depersonalization, nocturnal desaturation and an elevated respiratory disturbance index (RDI) are all consistent with the diagnosis of obstructive sleep apnea.  The data does warrant your making a maximum effort to use the continuous positive airway pressure (CPAP) device effectively.  The diagnosis will be more certain if and when you are consistently able to use the CPAP effectively, with relief of symptoms.

The best recommendation we can give is that you work closely with the sleep lab technical staff in an attempt to make yourself more receptive and better able to cope with the CPAP discomfort you are now experiencing.

Treatment results:   During the night, the pt was noted to have OSA with an RDI of 23 prior to CPAP.  Nasal CPAP at 11 cm reduced his RDI to 2.7.      Lowest desat w/ CPAP was 91.0
