Small red/pink spots under foreskin

6 weeks ago I had unprotected vaginal sex.  I tested negative for chlamydia and gonorrhea at 5, 16 and 26 days. Negative for HIV and syphilis at 26 days at a GUM clinic.

About 10 days after sex I saw a tiny red spot under my foreskin (the skin, not the glans).  It cleared up in 3-5 days, but since then I have had a succession of similar spots.

A couple (2-4) will appear at the same time, fade over 3-5 days, then I will have 2-3 clear days before noticing more.

They are not 'clustered' or joined and the skin around them is normal. They tend to be spaced by a couple of milimeters to a centimetre when two or more appear.  I would say, however, that each episode has had it's own 'focal point' and it appears each episode has 'walked' relative to the previous episode so there is an overall movement from left to right 2-3cm.

The spots are tiny <1mm and start pink/red, fading over time.  They are too small for me to tell if they are blisters, if the skin is stretched they can appear to be domed (like a face pimple) but sometimes they look more like a rash, while still having the 'spot' like shape.  I have not noticed the top come away from any of them, or any ulceration - though some are so small I'm not sure I would be able to tell.

Occasionally there appears to be a corresponding spot on the glans that matches roughly with where the foreskin spots lie, though this is very difficult to make out and disappears much quicker.

No itching or discomfort. I went to a GUM clinic, but it ended up being one of the days there were no visible symptoms and was told it might be molluscum, but probably irritation from soap (I have not lately changed soaps).

There appears to be some ascetic whitening.

I used Canesten HC cream for 5 days, stopped 4 days ago when symptoms seemed resolved, but today I see two new spots.  I have taken a Canesten oral pill today.

Can you advise?

Thanks for your reply.  Last question - I appreciate you cannot make (or rule out) a diagnosis via this interface - but does the history of these lesions suggest it could be herpes or warts?

I was thinking that because they are so transitory they could not (or are highly unlikely to be) warts.

I also thought the movement/spread over time sounded unlike herpes.

I will try to show them to a doctor - I am aware of your practice/surgery in London and may come in for a full screen in another 6 weeks.

Thanks again.

Hello,
Thank you for your post and welcome to our forum.
It is certainly very difficult for me to make a diagnosis without examining you properly and my advice to you is that you need to be seen by a doctor whilst having symptoms.
My impression from your description is that it is some kind of balanitis.  Balanitis can have many causes, including fungal infection, and it is possible that this could be the cause as it responded well to Canesten HC.  It could also be caused by a skin condition, like dermatitis.
You need to be exmained whilst having symptoms, and possible get some further tests.  Until we do not know what the cause of the problem is, it is advisable to abstain from unprotected sex.
Kind regards,
Dr José
