Internal Anal Itching

I am an persistent itch inside my anus.  Sometimes the itching is intense and I will insert my finger inside my anus.  The probing  stops the itch.  When I remove my finger, it will have dead skin on the tip of my finger.  This problem started about 3 weeks ago.  

I am a sexually active gay man who has unprotected anal and oral sex on a regular basis.  Lately, anal sex has been very painful and I usually have to stop anal sex after a few minutes because the pain is too intense.  I am HIV+ and I am not on HIV meds because my T-Cells are very high (800's).   My viral load is also very low (10,000).  I get my blood work checked regularly at an HIV clinic.  In the past 6 months, the doctor did an anal swab to test me for Chlamydia.  The results came back negative.  

I also experience intense itching on my balls and genital area.  For over 1 year, the doctor prescribed ketoconazole and it does not seem to work.  It will stop the itch for a few hours but it returns.  My balls are red and I have red lines in the cracks between my hips and groin area.  Recently I purchased Lotrimin Ultra (butenafine hydrocloride cream 1%) and it works for about 12 hours.  

In conclusion, I have problems with internal anal itch and jock itch which does not respond to jock itch creams.

Internal Anal Itch:   The itch is about 1 inch inside the anus.  Sometimes I insert a finger to scratch the itch.  When I remove my finger it always had a ball of dead skin on my finger.  I also have intense pain during anal sex.

Jock Itch:  My doctor put me on Valtrex and the itching does not go away.  I might need to see a different doctor.  I need relief from the jock itch.  I've had this for about 2 years and the ketoconazole is not effective.  

Question:  From your experience, does it sound like I have some type of STD that is undiagnosed.  When I do my HIV bloodwork, all STD's come up negative.  Please help guide me in the right direction as I need relief and a cure for what ails (itches) me.

It might be pinworms in your anus.  The itching is abosolutely out of control.  Parasites also give you that type of anal itch. They say woodworm tea is helpful.  Parasites use you as host and spread throughout large intestine.
Pinworms are the most common worm infection in the United States. They are most common in school-age children.

Pinworm eggs are spread directly from person to person. They can also be spread by touching bedding, food, or other items contaminated with the eggs.

Typically, children are infected by unknowingly touching pinworm eggs and putting their fingers in their mouths. The eggs are swallowed, and eventually hatch in the small intestine. The worms mature in the colon.

Female worms then move to the child's anal area, especially at night, and deposit more eggs. This may cause intense itching. The area may even become infected. When the child scratches the itching anal area, the eggs can get under the child's fingernails. These eggs can be transferred to other children, family members, and items in the house.

Symptoms
•Difficulty sleeping due to the itching that occurs during the night

•Intense itching around the anus

•Irritability due to itching and interrupted sleep

•Irritated or infected skin around the anus, from constant scratching

•Irritation or discomfort of the vagina in young girls (if an adult worm enters the vagina rather than the anus)

•Loss of appetite and weight (uncommon, but can occur in severe infections)

Not really sure what it could be. I would suggest seeing a new Dr for this issue.

Also you say you have unprotected anal sex...I would assume that it is with a partner who is also HIV+?
