many things cause ngu and prostatitis, not just chlamydia. you haven't been properly treated for most of the causes of either of them hence my recommendations.

Zithromax 1gm wouldn't that in itself rule out prostatitis and gnu due to chlamydia being treated?
you can get chlamydia and gonorrhea but oral sex but the risk of such is very, very low. treating you without testing you was irresponsible also.
at this point I wouldn't worry any further about the dose you were given ( 1gm is the dose but just as a single treatment isn't recommended ). Instead, if symptoms continue/return, be seen and get basic urine testing done for a proper diagnosis. It's far more likely to be prostatitis or ngu than gonorrhea or chlamydia anyways and one dose of azithromax isn't very effective for either of those.
