gonorrhea or chlamydia Or something else?

About 3 or 4 days after unprotected sex with a female, I noticed burning during urinating and a tingling feeling around the head of my penis. I also had a discharge coming from my rectum not my penis. It took 2 weeks to get to a doctor. They gave me a 1g dose of azithromycin. The symptoms seemed to get better for a day or 2 then came back worse than before. Went back to dr a week later. got 10 days of doxy. Now my left testicle has had pain (epididymitis) dropped way lower than the right. Finished the doxy. Went back got a shot of ceftriaxone. Day after that shot I saw the girl again. We did not have sex but did kiss and use hands. Symptoms remained. They got worse about a week after that. Got another dose of azithro and doxy. No change. Now a full 2 months later symptoms are still present. I have never had a discharge from penis, only rectum. There is a slight discoloration on the penis head. No sores blisters or anything else. This is 3rd time I've had these symptoms in my life all after unprotected sex. Doxy cleared the first 2. Ive had 4 urine tests all shown clear and fine. I am currently on day 6 of 10 of levofloxacin and no change now. Any ideas?

In doing these tests, did they do any STD testing?

If you have a rectum discharge, have they cultured that? Has anyone checked your prostate?

Yes it was for gonorrhea and chlamydia. No swab tests as the 3 dr I've seen all say they dont do that. Yes i did have a prostate exam. All fine.

Are you seeing a urologist? If you've been diagnosed with epididymitis, you should be under the care of a urologist.

When you ejaculate by yourself, do you get these symptoms, or is it only when someone else is involved? The only real difference in that might be the duration of activity, because the process for your body is the same. Since kissing and hands don't transmit anything, and your symptoms got worse, it makes me think not bacterial (and you've been on a ton of different antibiotics at this point, anyway).

I would think a urologist would swab the rectal discharge, though I don't know how accurate it would be while you are on antibiotics. The pain from epididymitis can last a long time - several weeks. Did you have the discharge before starting the antibiotics? Some antibiotics can cause that.

Oh and even if your anal discharge was caused by an STD - which can sometimes happen even if you haven't had anal sex, you would still test positive for them on the urine test.
Yes discharge started before antibiotics. I am under 3 urologists. They are clueless here. Its really frustrating. And the fact that none of them swab anymore is even more frustrating and ignorant i think.. Especially since I've had this same exact thing 2 times in the last 10 years with identical symptoms and doxy cleared it up both times within a week. My worry is it is a resistant strain of gonorrhea and prolonging this is making it worse.

You tested negative for gonorrhea, so that shouldn't be an issue. Even if it's resistant to antibiotics, it would show on a test.

My question now would be your testing times in relation to your antibiotics. When you did the STD testing, had you already taken any antibiotics? If you did, you could be getting false negatives.

When you are done with all the antibiotics, you could go to an STD clinic, tell them you had unprotected receptive anal sex, have an anal discharge, and they will likely test you anally. You might have to go to your closest larger city if you are in a small town.

It would seem unlikely that every new partner you have has an STD, right?

The first test was not on antibiotics each one since has been or right after. I saw a thing that said the urine tests should be done first thing in morning or if you havent urinated for 2 hours. None of my tests were done that route. I also thought the same that it's unlikely 3 times in my life that a new partner had this. Just a side note none of these 3 were strangers they were all friends. Doesnt mean they didn't have anything but if they did they sure didn't know it. I find it very strange though first 2 times doxy cleared it and this time it didn't.
Yes, it's important that your urine tests are at least an hour or two since you last voided, and it should be the first part of your urine stream.

When they did the urine tests, did they ever mention white blood cells in your urine?

I still wonder about your prostate, but that shouldn't only coincide with sexual activity with a person.
They said white blood cells were normal. I saw another urologist today that told me I was basically making it up. Im tired of being ignored here. A discharge out of my rectum constantly and a low hanging painful left testicle,, with off patches of pinkish off coloring on my penis is not normal.. being told my symptoms don't line up with anything is just absurd. He gave me a shot of ceftriaxone and said if that makes me feel better. I also finished the last levofloxacin tonight. Tomorrow I have an appointment to get a swab test done. I'll go from there I guess
