Help - Questions on Chlamydia / HIV

Dear Doctor

I've been in a monogamous relationship for 3 years and I recently had sex with a co-worker who is 25 years old. She is caucation, from Australia working in the UK, she is heterosexual as am I. It is the first time I've had sex outside of my relationship.
2 weeks after we had sex, I developed symptoms associated with chlamydia/gonorrhoea and got tested. I was positive for chlamydia but negative for gonorrhoea. I was treated the same day I took the test for chlamydia (1g Azithromycin) and 2 days later I was treated with Suprax/Cefixime for potential gonorrhoea. My long term partner received the same treatment on a precautionary basis.

My symptoms cleared after a weak and its now 4.5 weeks after treatment. I re-tested myself last week for both Chlam/Gonorr with a home test which came back negative for both.

I'm not starting to feel generally unwell, I've been having strange pains in my legs and stomach too. I'm feeling more tired than usual. There are No signs of sore throat or flu like symptoms. My partner has also been experiencing the same thing. She also has a strong fishy odour from her vagina with slight white discharge. We've not had sex in a month and the fishy odour only started once we starting having sex again.

My questions are:

1. What are my risks of contracting HIV from one sexual encounter and should I be worried / tested?
2. What are the possible causes of this general feeling of being unwell and leg/stomach pains and could it be related to an STD?
3. Should I be tested for Trich and how serious is trich if left untreated?
4. I've started to get a little strange burn sensation when I urinate and wondering if I should be concerned that my treatment for chlamydia was not successful?
5. With the strong fishy odour, is this a sign of something more serious than BV?
6. Do you recommend being re-tested at a GUM clinic or to visit my GP?

Thanks for your quick reply.  

A few last questions:

1. What are my chances of having contracted Hepatitis (B) and do my symptoms align with possible infection or is it uncommon to attract Hep B in the UK?

2. As its only 4.5 weeks after treatment, do you recommend waiting until the 6 week point before being tested again for all STDs including HIV and Hep B?

3. Would prostatitis / other non-STDs be a concern at this stage?

Thank you very much for your post and welcome to our forum.
I will answer each of your questions here below:

1. What are my risks of contracting HIV from one sexual encounter and should I be worried / tested?

THE RISKS FOR YOU ARE VERY LOW.  FIRSTLY IT WOULD BE HIGHLY UNLIKELY THAT THE GIRL WAS POSITIVE IN THE FIRST PLACE.  SECONDLY, EVEN IF SHE WAS, THE RISK FOR YOU WOULD BE LESS THAN 0.1% OR LESS THAN 1 IN 1,000.  I DO NOT THINK THAT YOU NEED TO WORRY.  HOWEVER A HIV TEST WOULD BE INDICATED BECAUSE IT WAS UNPROTECTED INTERCOURSE AND EVEN THOUGH THE RISK IS EXTREMELY LOW, THERE IS SOME RISK.

2. What are the possible causes of this general feeling of being unwell and leg/stomach pains and could it be related to an STD?

IT IS POSSIBLY UNRELATED TO AN STD.  HOWEVER A FULL STD SCREENING IS WARRANTED JUST IN CASE.  IT IS MOST LIKELY TO BE A VIRAL ILLNESS.

3. Should I be tested for Trich and how serious is trich if left untreated?

YES, YOU SHOULD AS SO YOUR GIRLFRIEND AS THE SYMPTOMS ARE CONSISTENT WITH THIS.  IT IS EASY TO TREAT WITH METRONIDAZOLE.  IF LEFT UNTREATED IT COULD POTENTIALLY CAUSE COMPLICATIONS.

4. I've started to get a little strange burn sensation when I urinate and wondering if I should be concerned that my treatment for chlamydia was not successful?

YOU SAID THAT YOU TESTED NEGATIVE AFTER THE ANTIBIOTICS.  IT COULD BE RELATED TO THE SAME SYMPTOMS AS YOUR GIRLFRIEND'S.

5. With the strong fishy odour, is this a sign of something more serious than BV?

NO, IT SOUNDS LIKE TRICHOMONA OR GARDNERELLA (BACTERIAL VAGINOSIS).  YOUR GIRLFRIEND SHOULD HAVE A VAGINAL SWAB AND BOTH OF YOU SHOULD GET TREATED WITH METRONIDAZOLE.

6. Do you recommend being re-tested at a GUM clinic or to visit my GP?

PREFERABLY AT A GUM CLINIC.
