STD question

Hi Doctor,

I had unprotected sex with lady, sex worker in Asia 6 days ago. I’m very concerned I have been experiencing the below symptoms since 3 days after the incident.
1. Burning, some itching, I see some what clear discharge but not a lot. Not sure if this is due to my continues examination, also it burns and hurts when I go to toilet.
2. Gender groin lymph nodes on my right side (the lymph is real and can feel when I touch)
3. Lower abdominal cramps
It’s been 6 days and I went to the clinic  today where they conducted regular “urinalysis” test which the doctor said all was normal:
Glucose was negative
Bilirubin was negative
Ketone was negative
Specific gravity was 1.010
Blood negative
pH: 7.5
Protein negative
Urobilinogen 3.2 umol/L
Nitrite negative
Leucosytes: negative
WBC had reading of .5 and the range supposed to 0-6.1
Bacteria was also 0.0

Sorry, I’m listing all of these results for second opinion because I’m  in 3rd world country and the doctor I have talked to is not STD expert.
Come down to my question, will regular urinalysis test detect STD’s?
I was so worried about the symptoms and asked the doc for medicine; he gave me zithromax which I haven’t started yet.
What is your opinion on the STD doctor? I will go for full blood test in few weeks. Thanks

Hi Mark,

The preferred combination to cover gonorrhoea and chlamydia is 1 g azithromycin and 400mg cefixime taken as a single dose.

Rather than diagnose yourself it always helps to actually see a doctor experienced in the area and have proper examination/investigation etc.

If this is not possbile for you then I would suggest that you attempt to source some cefixime 400mg and also some docycyline 100mg twice daily for 10 days. Be sure that you are not allergic to either drug and look up the product characteristics on the net so you fully understand the side effects etc before you take them. As I say, this is plan B. Plan A would be see an experienced STD doctor.


I would not take any more zithromax. Zithromax, whilst not a first line treatment for gonorrhoea does have pretty good efficacy against it.

It seems unlikely that gonorrhoea would be a possibility here since your urine was clear - and gonorrhoea produces large amounts of copious pus cells usually.

Why do you need friends on standby - I didn't understand that bit.

I have taken 1g of zithromax on Friday and still having symptoms especailly burning when I urinate. I read that 1g of the zithromax will not cure gonorrhea, i have taken i dose of 2g zithromax today which is Sunday couple days after the 1g that I have taken on Friday.

Is that safe or will it harm me? I'm working in Asia and home alone without family and concerned. Should I ask some friends to be on standby?

It sounds distinctly possible that you have acquired an STD/STI and so I would take the zirthromax - you haven't said what dose he prescribed but I would suggest 1 g zithromax as a stat dose - you would need to consult locally though with the original or another doctor.

The results you have quoted seem innocuous, but the fact that you have dicomfort and a possible discharge indicate that you should take the medications pending further more specific results.
