This is what you get for being stupid and drunk

Dear Doctor,

I recently(21/06/2010) had unprocted sex with a sex worker while drunk. I ejaculated inside her and also kissed her. What they call a girlfriend experience. I am 25 year old male, uncircumcised. I washed my penis twice after the encounter, as if that helps.

Last night I felt a tingling sensation at the tip of my penis. After the unprotected sex I immediately went to the bathroom to check it out. I noticed that there was a white discharge from the tip of my penis. After urinating I started to notice a mild burning sensation at the tip of my penis. To make sure I had no other symptoms I checked the head of my erect penis and saw some red spots(extremely small) and redness on the head. The opening at the tip of my penis feels raw as well.

First thing this morning I went to my local GP and told him of my symptoms. He did not examine my penis, but did examine my throat as I was complaining of a raw throat as well. He did prescribe 750mg levofloxacin and 200mg clotrimazole. Seeing as how I do not have any health insurance and the state run hospitals are pathetic, he said that a blood test would be quite expensive and that I should take the medication and report back to him if my condition did not improve.

I then notified this sex worker that she should get herself tested and that she has most likely given me a STD. She replied saying that I'm lying and that it's impossible, she gets tested montly(her last test was 19/06/2010), and that she has no STDs. I do believe they get tested montly as the place where she works is quite respectable. Somehow I don't believe her. During the encounter with her I was fingering her when I felt that she had an enlarged cervix. She also got extremely wet. Her fluids were the natural colour and I wasn't worried at the time.

Either this whole situation is one large coincidence or she has an STD and refuses to admit it.

From my symptoms described here would you agree that I most likely have a bacterial STD?

Dear Mr Muffzy,

It is likely that you did indeed contract some sort of infection from this lady although we don't know what.

It is probable that given your symptoms you had either an NSU or chlamydia. Both can be caught sexually. To treat chlamydia the ideal antibiotic is azithromycin or doxycycline. You will need to ask your doctor about doses and supply - I am unable to do that here.

Although the lady is tested every month she may have been infected immediately after her last test and then passed it on to you. She may well not know about it as up to 70% of women with genital chlamydia for example do not have symptoms.

I was uncertain about the large cervix observation - it is in any event irrelevant to your infection.

So, I suggest you stop persecuting the lady you had sex with but rather go back and see your doctor and ask about taking azithromycin or doxycycline for what seems most likely to be chlamydia.

Thanks for the help. I am glad to report that after 4 days of treatment all my symptoms have disappeared. I will continue the antibiotics and the anti-fungus treatment though.
