A sex worker is trying to protect herself from you, and her other clients. She isn't so much concerned about your health, or trying to protect you, but herself.
Why would she poke holes in a condom, taking the chance of infecting herself? Even if you think she has something she's trying to infect you with, which isn't a rational thought at all, she wouldn't take the chance of infecting herself with something else.
Her job - her livelihood - depends on her safety.
In any case, you ejaculating doesn't change your risk. It could change her risk, but not yours.
So did you have protected oral and protected vaginal? If you did, you are totally protected from HIV, chlamydia, gonorrhea, trich, NGU and mycplasma. There is a very, very small risk for syphilis, herpes and HPV. The risk is low enough that I wouldn't worry about any of it without symptoms.
Confused what your actual problem is, as I can't envision a woman doing this and then having vaginal sex with you. So is this something that you know happened, or are you actually suffering from irrational anxiety? Because if she was going to have vaginal sex with you, or if she was using this to protect herself before performing oral on you, why would she then do something that would defeat the purpose of protection from a cond0m?
