Anxiety or Real?

I am a 45 year old hetrosexual male.
On Nov 4th I had unprotected oral and protected vaginal sex with a commercial sex worker in Persia. I was intoxicated and the following morning I checked the condom and it was torn. I immediately freiked out.
Noticed tenderness in testicles and slight pain when urinating.
I had my 500mg amoxicillin tablets with me. I started taking 1500mg X 2 daily for 5 days.
Noticed frequent urination
I went to the doctor (locally) on the 10th of Nov and was given Ciprofloxin and Zithromax.
Taken as following schedule:
Nov 10   1000mg Ciprofloxin & 1000mg Zithromax
Nov 11   1000mg Ciprofloxin & 1000mg Zithromax
Nov 12   1000mg Ciprofloxin
Nov 13   1000mg Ciprofloxin
Nov 14   1000mg Ciprofloxin
Nov 15   1000mg Ciprofloxin
Nov 16   1000mg Ciprofloxin
Noticed Diarrea and frequent urination.Lower back pain and abdominal tenderness. No discharge
I returned to the US on the 16th of Nov.
Noticed small skin rash on left knee and dry skin patched on cheeks.
Nov 18th attended appointment with family doctor. I told him I had been traveling and had diarrea and abdominal tenderness and frequent urination. I asked him to perform every test possible. He performed blood work, urine, and stool. Told him about antibiotics taken while out of country.
Result = nothing abnormal
I have never had discharge and no visible signs of STD other than rash on knee and face. No fever, no night sweats, and no discharge.
However, I seem to have increased sensativity in penis head and increased frequency of urination, and occasional shooting pains in euretha. No sure if this is due to anxiety.
What would be your opinion of having:
1) Bacterial STD?
2) Viral STD?
3) Could all this be caused by anxiety?
4) Is there a STD that displays the symptoms I have presently? (Penis head sensativity, frequent urination, non genital rash)
5) What do I do next?
6) Am I being over paranoid?
7) Could this now be yeast infection due to antibiotics?

Time has a way of healing even the overactive imagination.

I have a appt with a discrete lab testing service tomorrow for added reassurance.

The symptoms you describe really do not sound like an STD, bacterial or otherwise. The antibiotics you have taken would have prevented or cured all common typical STDs such as gonorrhea, chlamydia, nongonococcal urethritis, syphilis, etc.  As for viral STDs, what you describe is not suggestive of a viral STD.  Indeed anxiety could be a major contributor to what you are experiencing.  we all have sensations all over our bodies, including in our genitalia which we tend to  ignore and/or overlook until we have an exposure or experience such as yours which causes us to worry.  You have taken effective antibiotics, you have been examined and your tests are negative.  It is time to try to relax and realize that you do no have an STD.

As for your diarrhea, the antibiotics could certainly cause it, as could something you might have picked up during your travels although the negative tests makes the latter less likely.  The antibiotics might have also set you up for a yeast infection but without a rash or some other visible abnormality, this too is unlikely.

My advice - relax and try to accept that as concerned as you are, the fact of the matter is that you have managed to get through the exposure you describe without having gotten an STD  If my reassurances are insufficient, I would then suggest you either return to your own doctor or see your local STD clinic and tell them the WHOLE story. I am confident they will come to the same conclusion that I have.  Hope this helps.  Good luck.  EWH
