sore red penis and painful scrotum -worried

I am a male and received oral sex with a sex worker 6 weeks ago and starting 10 days ago my head of my penis has been red and sore and itching occasionally(I wore a condom but touched her vagina several times and worry i touched my groin with her fluids at some point maybe taking off the condom). I had a full bill of clean health up until now and married for many years with no promiscuity until this incident. I went to the doctor a week ago and he said it was nothing and it was my guilt. I looked normal to him but my testicles sound like the other gentleman's problem. something is not right. My testicles are red, tight, and seem to burn occasionally. Last night i masterbated (I am circumsized) and the ring around my head of my penis became sore and red and throbs now. I have no lesions, or spots. But I am very afraid. could this be scabies? herpes? exzema, fungus or another std? is there anyway I could have got HIV? I feel feverish and awful but I have been panicking for two weeks. I cannot tell my wife or she would leave me and I am trying to not panick but I cannot concentrate on anything but this. Please kindly help me. I really appreciate it. thank you.

You are over thinking this. Your exposure was low risk and the scenario you describe is simply not realistic.  Nothing to worry about, as I said.  EWH

thank you so so much. please just reassure me that I could not get HIV from this incident. I am freaking out and now starting to feel feverish and sick. I keep going over the event again and again and remember that she put lubricant on her vagina expecting me to penetrate and then she put condom on me - (then i said i did not want intercourse - I was horrified) and then she performed oral sex -  could her finger have passed something to my urethra when she put the condom on me? Could it be possible after i came and took condom off I was my hands - but if i touched my bottom later when I went to the toilet could I have transfered HIV virus into me? please kindly reply.

On occasion small amounts of blood are normal.  Don't worry and let's see what the tests show.  Good show for going to your doctor.  It was the  right thing to do.  eWH

Thank you so much for replying. I saw my doctor again this morning and he thinks it is fungus but not sure. he said the same as you but for piece of mind i did all std tests and results will be in 3 days (I know i am going to panic non-stop and will try and do everything to keep my mind off it and take will try and recite yours and my doctors prognosis to help calm me. When i gave the urine sample this morning i saw a drop of blood in my urine. That has added more concern and worry now. What could that be?

I am going to largely confirm what your doctor has said.  The scrotal symptoms you describe are not suggestive of an STD of any sort, nor is the irritation you have experienced following masturbation nor your feverish sensation.  Your exposure was condom protected and this is safe sex, despite the fact that it is entirely normal and expected that there might be modest exchange of secretions from one persons genitals to the other’s skin during the experience.  Similarly, HIV is not a concern.

In addition to the fact that your symptoms are not suggestive, the timing is really all wrong. If you had gotten an STD from the exposure you describe the symptoms would have begun earlier than what you describe.

As you point out, you have been concerned about this for a while. In these situations, it is not at all uncommon to become "hypersensitive" to sensations that really are quite normal but are dismissed or overlooked when one is not concerned that they might have a problem.  I would urge you to try to not worry.  EWH
