NSU / Herpes

March 2007 I had safe sex. In April I noticed wetness after peeing and peeing more often. Doctor did swabs twice  for chlam, gon etc -  all clear.  Gave Zithromax said maybee an NSU.   In July I had a safe sex. Wetness continued. Went to another doc-  took urine samples -  found nothing.  He said I had  thrush, gave me cream & said see a consultant who found nothing, said I might have balanitis .  He said that the wetness was probably from me not emptying properly as it came after peeing. Also went to a GUM.  They did all STD tests and said they found an NSU but  clear for all others. Gave me Zith.  Took it on  20 Sep as I was waiting for consultant. 2 weeks after I had safe sex.  After noticed a clear discharge from my penis when erect.  2 weeks later went back to the GUM clinic.  They tested me for NSU and said it was clear.  They also did a herpes swab test - clear.  Said that the discharge sounded normal.
I had safe sex on 24 Oct.  The clear discharge continued (not just linked to peeing) GUM again tested me for NSU - doc said that he found a small number of 'pus cells' on the swab  -sent off urine analysis for chlam and gon –result clear.  Since swab 10 days ago I have again noticed a clear discharge-not just when erect. I had 3 spots at different times in Nov in dense pubic hair -each connected to a hair - no pain.  I read somewhere that that is an unlikely place to get herpes spots? No spots on penis outside or inside.  At the opening my penis seems red but no one who has examined me has siad anything about it - had ingrown hairs before.  I have been examing my thighs a lot - knees to groin, and have noticed single small spots which appear to have a solid centre - like hair follicules.  They don't hurt and if I pull them the base of a hair seems to come out. Can you get herpes and not have sores etc on the penis or scrotum but have them on the thighs? What about the 'small number of pus cells' that the GUM clinic decided not to treat and the discharge?

Testing for trichomonas is difficult in men.  the recommendation is just to try treatment.

Regarding herpes, although it can occur in many locations other than the penis, do not be concerned, what you describe is not herpes. EWH

Sounds like there are several issues to discuss here.  Let's start with the discharge from your penis.

You have been told several times the you have NSU or, as we call it here in the US, NGU (non-gonococcal urethritis).  Whatever you call it, it is probably the most common symptomatic STD in men and can be caused by a number of different bacteria, including chlamydia.  This would have given you what the health care provider called "pus cells" on the swab.  As for the number, a few such cells may be normal but more than a few is not and warrants treatment. Assuming that is part of what you are experiencing, then what could be causing NSU which did not respond to azithromycin which is curative for most NSU?  Perhaps the most common cause is trichomonas.  We recommend re-treatment with azithromycin or doxycycline PLUS a single 2.0 gram dose of metronidazole in these situations.  The trichomonas can cause NSU as well.

Also, several of the "discharge events" you describe do sound normal, including having a small amount of residual moisture or liquid present after voiding or accompanying erections.

Now for those "spots" associated with pubic hairs.  What you are describing sounds like folliculitis.  These are not an STD and are common and respond to soap, water and a wash cloth.  Ask your GUM provider about them however since trying to make a diagnosis over the internet is not something we recommend--you need an examination.

Hope this helps.  EWH

Dude I have had the same dam thing over and over again. I had safe sex with CSW. I had clear discharge and went to the doctor and he told me I was negative for both Chlamydia and Gonnorrhea. He told me with an intact condom theirs no way I could have contracted any NSU. Maybe our clear discharge is pshycological or maybe even normal mucousal secretions or even precum. I also heard of urethral irritation(look it up).

Thank you for your reply.

Should I specifically ask to be tested for trichomnas?  The provider did think about giving me the doxycycline but decided against it.  Also is it normal or usual to have herpes but not to have it on the penis?
