Follow up

doctor I am following up from my last posts over the past couple of weeks.  As stated I had unprotected intercourse with a woman a month and a half ago and again about three and a half weeks ago (several times in one night).  about a week and a half after the last exposure I noticed a red area  a few hours after masturbatibg that looked irritated under the head of my penis on the right side.  I went to the dermatologist who said he wasn't sure but didn't think it was hsv.  I used a topical cream for irritation he have me and the area becam normal again in a few days.  I therefor thought it was just an irritation, perhaps from pulling the skin when masturbating. Then, again several hours after masturbating, I noticed what looked like a very small cut, in the same area where the redness had been.  the next day after masturbating again the cut seemed to become larger and more circular.  It's small but relatively noticeable.  Also, the skin just above the cut on the head of the penis seems to be darker than elsewhere, although there is no breaking of the skin there.  This lesion first appeared about 18-19 days after the last exposure.  I don't see any other lesions.  At first I though it was another irritation but I am concerned that it has not healed yet.

does  this sound like hsv?  
is it too late for a swab test?  
should I get a blood test and if so when?

You need to ask your doctor, or a dermatologist what the lesion might be. this can only be done with an in person exam. We do not make diagnoses over the internet.

only about a month after exposure, but given the PA's asssessment that it wasn't an std, I feel better about this.

I have two questions now:  if the lesion isn't hsv, what could it be?  I know it could be from trauma, but it hasn't fully healed almost two weeks after appearing.  Also, I don't understand why the skin just above this lesion (on the "lip" of the head of the penis) is a darker color than the rest of the lip.

Also, I did experience some pain in what seemed like my urethra over thr past couple of weeks.  The pain intensified early this week but has gotten better over the past couple of days.  It's a dull but somewhat constant pain sometimes in the genital area, but also in the area of the urethra between the testitcles and the anus.  At first I thought possibly it could be a uti, but the doctor said the urine test would have picked that up.  I know u have said some of this could be due to anxiety, but I am farirly positive the pain is real.  It's get worse when I sit for long periods of time.  It doesn't hurt when I urinate.  Again it has improved over the past couple of days but it is still there.

I went to the doctor and had her look at the lesion.  Note the doctor wasn't in and a phaysicians assistant saw me.  She said it was not an std and told me to apply cortisone cream.  She also took my blood and urine for tests.  The blood tests came back negative for all std's.  Urine test did not show any irregularities.  I understand the blood test wud have a good chance of not picking up hsv

Thanks for the response.  I want to be clear that this lesion is different than the prior redness. It began as what looked like a smal cut, and it has increased in size, although it is still small.  But it is broken skin, unlike the first red area which had no broken skin.

.I'm sorry you feel the need to keep re-visiting this topic.  Before I address your continuing concerns, let me point out first we typically limit the number of responses to questions in order to allow us time to address the concerns of others in the limited number of spaces/time we have.  In your case, your hyper vigilance lead you to repeatedly ask the same questions again and again.   I would point out that it was Einstein who said " To repeat the same question over and over and expect a different answer is one definition of insanity."

Second, also to allow us to handle the questions of most who seek our input, we ask clients to make only two posts in any six month period. This being your second post, I will ask you not to ask new questions once we have worked through this question until at least July of 2012.

Now for your continuing concerns.  You experience recurring redness in an area of previous trauma following masturbation. This is not unusual or unexpected.  Areas which have been traumatized (or have had open sores) are more vulnerable to recurrent irritation than untraumatized skin.  In your own case, I suspect that your anxiety and guilt-driven hypervigilance has also contributed to concerns and perhaps to noticing areas of redness which you probably would not have noted prior to your indiscretion.  

With regard to your specific questions:

1.  does  this sound like hsv?  
No, this still sounds nothing at all like herpes.

No, this still sounds nothing at all like herpes.

2. is it too late for a swab test?  
PCR or culture tets can be done at any time including on intact skin. They do not require fresh lesions or blisters to be performed although some health care providers are under the misconception that this is needed.

3.  should I get a blood test and if so when?  
My answer has not changed.  Blood test are a waste of time.  You could just as well burn money for its value to you.

If my answers seem terse, I apologize.  I have and continue to provide you with the best possible answers.  On the other hand, asking the same questions again and again are not productive.  Have you considered discussing your concerns and guilt with a counselor?  EWH
