Gave & Received Oral Sex, Sore Throat, Urethra Irritated - Might be in my head.

Feb 6, 2008 I gave oral sex to another guy, and he did the same to me, both to completion, no condom.  6 days later I got sore throat, mild in pain but lots of white stuff and holes on tonsils. My bros and I all get strep all the time (i'm 26 now), so assumed that's what i had - went to doc, she did a strep test and said it was neg but treated me with a drug called amoxil, it was 875mg twice/day  told her about the oral sex, so she had me take 3.5pills the first day and then finish the course normally.  throat looked better, still tiny bit sore in 7 days. I am seeing a girl here, we have protected sex though our condom did break once and we replaced it after realizing - maybe 2 minutes later. This was about 2 days after starting the amoxil. we had protected sex a couple more times over next 2 weeks, and UNprotected sex around maybe the 20th of feb give or take a day. Beg of feb my girl had a uti and took macrobid which she had a bad reaction to. they gave her a 'z-pack' instead on feb 11. Beginning of this week had a tiny bit of discomfort in my urethra tip, nothing major. As precaution went to same doctor - she looked and said she didn't see anything and declined to do std tests. no discharge of anykind at any time of day, except pre-*** when i masturbate. The pre *** is slippery, then gets slightly sticky after a minute or so - looks and smells normal to me. urine is clear to.  She (doc) then gave me a 1 gram dose of azithromycin to "kill my fear" and wrote 'urethritis' on my paper, no followup.

That was monday, it's now thurs and the urethra feels the same. slightly irritated, no discharge of any kind. understand oral is very low risk. without a discharge, i am clear of any std?  was my sore throat possibly related to oral sex (giving or receiving)?  are my symptoms this month connected, and if so why so long in between sore throat and irritated urethra? if std possible, is my girl at risk even with all the drugs weve had? (she has no symptoms at all to date)?

Sorry about the typo.  You have it right:  "likely you aren't" or "unlikely you are", take your pick.

I don't care what websites you have seen, azithromycin is not given with any frequency for UTI. Eighty to 90% of the bacteria that cause UTI are not even susceptible to the drug and virtually any doctor would know that.

By definition, NGU occurs only in men.  In some cases, the female counterpart is cervicitis, especially with chlamydia and perhaps with other causes.  But nothing abnormal if found in most partners (female or male) of men with NGU.

I agree with your closing conclusion, and see no reason to worry about protected sex with your girlfrend.  Whatever either of you had, most likely it's gone.

Initial reaction, only from the title of your thread, before I read the question itself:  Oral sex is low risk for STDs, so on that ground alone it is unlikely (but not impossible) that you aren't infected.  Second, whenever a person suggests his or her own symptoms "might just be in my head", usually s/he is right.

OK, now I have read the question.  It's not quite as simple as the above comment suggests.  Most likely the initial throat infection was due to a garden variety respiratory virus, or was a strep throat despite the negative test for it, and has nothing to do with the mutual oral sex event.  But it could have been gonorrhea or herpes.  However, oral gonorrhea usually causes no symptoms and initial oral herpes usually is more severe and lasts longer than you describe.

However, I am confused by your female partner's situation.  Azithromycin (Z-Pak) is never given for UTI, and that treatment suggests her doctor suspected an STD, especially chlamydia.  Chlamydia could explain your urethral symptoms as well.  However, you can't catch chlamydia by oral sex -- but conceivably that problem started with your female partner and has nothing to do with the other guy.

Or perhaps you indeed caught nongonococcal urethritis (NGU) from the oral sex event. Sometimes that can be due to normal mouth bacteria, in which case there was probably no danger of transmitting anything important to your female partner.

At this juncture, you're probably never going to know for sure what went down.  It is too late for further testing, since you and your girlfriend have had antibiotics that would have cleared up chlamydia, gonorrhea, or the other bacteria that cause NGU.  Whatever you had to start, I suspect that your continuing symptoms indeed are just heightened awareness, not relfecting any continuing problem.  However, If your symptoms persist or recur, return to your doctor.  It sounds like she is pretty well up to speed on STD care.  In that event, also bring your girlfriend to be checked at the same time.  But most likely you're done with all this.

I hope this helps.  Best wishes--  HHH, MD

glad i caught that! reading some of your other posts it would seem to be too long to be ngu, but just in case - how long should it take the antibiotics to clear it if it is that? a week? 2 weeks?  how long should i wait before worrying again, if ever?

wait, was that a typo?  you wrote "so on that ground alone it is unlikely (but not impossible) that you aren't infected." - did you mean it is unlikely that I AM infected?

just a quick google search for uti z pack turned up tons of sites listing a zpack as a treatment for uti, but not the most common one.  so you might be right, maybe her doc suspected something else and just didn't want to worry her about it, or maybe she did worry her about it and my girl didn't worry me about it.  i'm not worried about chlamydia after searching this site, i've had 2 large doses of antibiotics so if that was a possibility as you mentioned, it's a dead one! had chly on my 1st sex experince at 16 - i remember that pain WELL, this isn't even 1/10th of that. i didn't consider the alternate of catching it from my girl, and not the one-time oral with the guy.

searched ngu and have seen tons of posts from men, but not many from women. if i did get ngu from oral sex, wouldn't i have a discharge of some type? are you saying even if my girl received this bug from me it would just disappear on its own, or that because she had the zithro-drug it would have been killed and thats why she has no symptoms? its been weeks, i feel like she'd have known by now if she had something after her antibiotic.

do i need to refrain from protected or unprotected sex, or oral sex, or masturbation while my urethra goes to normal - i saw a few threads with some complaints that alcohol, ejaculation, etc causes the thing to stay irritated - would rather hear it from a doc though. i'm ok doing what i need to on your advice to get to 100% instead of the 95% i'm at now.  or a simpler version of those questions - how long does it take for urethra to calm down, not infect other people if i did have ngu?  this is 3rd day after my 1 dose of antibiotic, do i need to wait or is that long enough?

after reading your answer, i'm about 90% convinced there has been no std transfer between the guy and me, or my girl and me, and vice versa. my tiny symptoms just dont seem to add up enough.
