Getting Herpes

I am not into oral, as I have a fear for the worst when it comes to STD's.  Back in April of this year I mutually masturbated with a good friend.  Obviously there were some vaginal fluids present but not as far as I am aware did they ever reach my penis as I was helping her as she helped me at the same time.  All was normal up until early Sunday morning 7/25/2010, I just had protected sexual intercourse around 5 AM.  I then decided to shower, and walked home, I noticed a bit of discomfort in my genital area around the base of the penis where the condom did not cover while I walked.  Later that day after sleeping I awoke around noon to use the bathroom I then realized that upon handling my penis there was discomfort around the base especially the top side of my pelvis where the connective tissue is located.  I noticed inflammation and redness 2 spots on the shaft about 1/4 of the way up and the others all at the base in 3' proximity. I went to the doctor and had him perform a culture as I await the results.  There are not obvious blisters just redness, feels tender, and itchy especially when exposed to heat.  Water seems to help with relief, as I have only tried mild cortisone ointments and they appear to having somewhat of an effect, but also cause a bit of discomfort at first.  It has only been just over 2 days and it appears to be going away slowly.  I contacted the partner and she has clean test. The spots on the shaft are almost already gone, just a little red.

Can hand contact transfer herpes form partner to partner?

Would rough sex lasting for more than three hours cause abrasions that may resemble Herpes?

Would the factor of the woman just finishing her menstrual cycle have anything to do with it?

Can condoms cause circular rashes around the base of the penis that may resemble herpes?

My doctor was unsure and decided to medicate me regardless. The rash is becoming much less noticeable and it only started sunday around 1pm, what can it be?

Welcome to the STD Forum.  I'll try to help.  

The exposures you describe do not support the possibility that this is genital herpes.  Herpes ins not spread by mutual masturbation and with respect to your exposure on 7/25, most importantly the time interval between intercourse and lesion development is far too quick to be newly acquired herpes and is far more consistent with a local reaction, perhaps due to vigorous sex or alternatively due to local irritation such as might occur with a latex (condom) allergy.  Furthermore the lesions do not sound at all like genital herpes and have gone away far too quickly to suggest  a first episode of HSV as well.  Having your doctor take a culture or PCR test from a suspected lesion was exactly the right thing to do to rule out the possibility of HSV if you were concerned and I am confident that you will have a negative culture, verifying that this was not herpes.  I would not continue to take anti herpes medications at this time.  Addressing your specific questions:

1.  No genital to hand to genital transmission of HSV, while perhaps theoretically possible, has not been seen and is not a concern.
2.  Absolutely.  Another possibility given the fact that the area of irritation sounds like it goes around the circumference of your penis is a latex allergy.  This could be ruled in or out by waiting until the lesions are completely healed (sounds like they are close), and then simply placing a condom on without having sex for about 15 minutes.  if this is a latex allergy the irritation will reappear in a day or so and you will know to avoid latex condoms (you could still use poly urethane condoms however).
3.  No, her recent menses should have no effect.
4.  See no. 2 above.  condoms could be involved.  About 8% of people are allergic to latex.
5.  See above. This is almost certainly not HSV.  I would stop taking the medication for HSV.

Thank You for the added peice of mind I will get into a dermatologist as soon as I can, as I am having skin issues in general from what I beleive is a sideffect of medication.

LAst time. This is not herpes. You do not need herpes medcation.  It may be cellulitis.  At the very least you need another opinion.  EWH

Ok I received the results of the swab test and they came up negative.  Most of the bumps have almost disappeared yet I still have a few.  Where the ones prior were located there is now a little red and irritated skin behind. Two of them got a little larger then formed a tiny black dot in the center; I believed this is an ingrown hair, or something like it. Also to note these were independent of the group and located within a 3" vicinity. Then yesterday upon coming home late and going to use the rest room I noticed the one on the lower shaft was now white tipped and sensitive, when I went to get a closer look and move the pubic hair out of the way it burst releasing white milky fluid, like that of a giant zit or black head.  After the milky fluid came some clear and then blood.  Naturally I cleaned it up, but have no clue what is going on please help as my doctor was even unsure, but according to the test not herpes.  What could it be and what should I do next?
