Gonorrhea, Chlamydia or NSU? I'm confused.

I am an attached man from the UK with some questions for you. I hope you can help me.

Wk 1:Received unprotected oral sex from a sex worker for a few minutes (She said she was tested 8 weekly and negative)
Wk 4:I gave oral to a man for 2 mins without ejaculation who is also clean
Wk 5:I was tested for gonorrhea and chlamydia (urine), syphillis and HIV- All negative
Wk 8:I'd had a pain in the head of my penis so went to get tested somewhere else. I was fobbed off and told I didn't need testing.
Wk 9:I had a weird sensation in my penis followed by one bit of clear discharge before bedtime. I had: throat swabs, urethral swabs, urine tests, anal swabs and blood tests. I was diagnosed there and then with NSU and given 100mg of doxycycline to take twice a day for 7 days.
Week 11: I had some yellowy white, cloudy discharge when I urinated in the morning.
Today (2 days later): All tests negative at clinic.

At around week 7 or 8 I noticed a few blotches on my glans with lack of pigment. I have no idea what these are and neither do any of the Doctors. I plan on getting to see a dermatologist via my GP in the near future.

I've had vaginal and oral sex with my girlfriend until week 8 but nothing since. I have no doubt that my girlfriend only has sex with me.

Now for my questions:

1): Is it possible I have given my girlfriend chlamydia or gonohhrea? The health worker said that it's not necessarily true that my NSU wasn't caused by her having either of them in which case I must've given it to her and my NSU was actually undiagnosed chlamydia or gonorrhea, twice.  I'd like to know if she's at risk of having either? I really thought that if I had confirmed NSU and not gonorrhea or chlamydia then she also can't have them.
2): If Herpes is the cause of my NSU (and white blotches) wouldn't it hurt?
3): Am I out of the woods as far as syphillis and HIV go?
4): Is there anything else that could cause my symptoms?

I'm at my wit's end :(

I cannot say whether or not prostate massage might have lead to some sort of more chronic irritation or not.  You might discuss this with your urologist. EWH

I'm so sorry to bother you again but I've just thought of some information which may be relative.

My initial exposure involved mostly prostate massage from the csw with only unprotected oral for about 20 seconds until I ejaculated. Would that have a bearing on what's unfolded?

Thanks.

Please re-read my answer (point number 1, original answer).  I said the only way to know for sure is for her to be examined.  Those findings would determine the need for treatment

Yes, persons can get recurrent NGU.  EWH

Sorry Doctor I just had another thought.

Does your comment on September 1st at 10:27 mean she doesn't need treatment?
If the cause is mycoplasma or ureaplasma can I get NSU again?

Thankyou very much Doctor. I'm glad I can finally breath a little easier.

When I see my girlfriend next I will go ahead and tell her about the NSU. I have learned LOADS from this terrible experience, most of all not to go after cheap thrills.

Thanks Doctor.

So to sum up is there 0% risk I've passed on Gonorrhea or Chlamydia to my GF and a real risk I may have passed on an unknown bacteria?

The tests are very good. Beieve the results of your negative tests.

Regarding your partner's risk from your NSU and what might have caused NSU, please re-read my original answer.

Trichamonas is not spread by oral sex.  EWH

Thanks for your reply Doctor. There's not enough room in the question field to be as concise as I'd have liked.

I forgot to mention that the same day as my last tests at week 9 (post NSU diagnosis but pre-treatment) I also did a postal Chlamydia test which came back negative.

I suppose what I really would like to know is: following 3 negative urine tests and 1 negative swab for Chlamydia, 2 negative urine tests and 1 negative swab for gonorrhea, can I be sure I haven't got or had chlamydia or gonorrhea and therefore passed one of those onto her or contracted one from her?

To try and put it simply is my NSU related at all to chlamydia or gonorrhea and is she/was she at risk for contracting either from me?

I didn't mention it in the first question but am I correct in thinking that Trichomoniasis cannot be passed via oral sex?

Thanks again. I hope I have managed to make a little more sense to you.

Welcome to our Forum. As much as I appreciate the detail you have provided, I also must say that I have had some trouble relating the timeline to your questions. I'll try to address them.

1.  Since you have not had sex with your GF since week 8, the question becomes, could you have had an STD before that time and given it to her.   Unlikely but cannot be 100% sure about this.   NSU can be caused (about 30-40% of the time) by chlamydia and is pretty easy to differentiate from gonorrhea.  Many other bacteria have also been associated to a small degree with NSU.  Whether or not men with non-chlamydial NSU can give women an analogous infection is not known.  Many experts think that there is little risk of sexual transmission of non-chlamydial NSU.  since there are so many unknowns and having said all of this, the only real way to know if she has something is for her to be tested

2. The white blotches do not suggest herpes and herpes is a very rare cause of NSU (<1% in most studies)

3.  Yes.  Syphilis would have manifested itself by now and HIV is not spread by oral sex.  I would not worry about either.

4.  Yes.  To really assess you should discuss with your doctor.  It is not normal to have a cloudy white discharge from the penis.  If urethritis is not the cause, the discharge could be due to prostatitis or some other process.

I hope this helps. EWH
