Had protected sex, and in need of some peace of mind.

Hi, I had sex with a woman I didn't know over a week ago. She took a condom out and put it on my penis when I gained an erection and that's when we started. She started by giving me some oral, applied some water-based lubricant then we had vaginal for a few minutes, and then finally oral again when I ejaculated. I kept the condom on the entire time. I did not perform any oral on her.

A few days ago my urethra started to feel irritated and when I looked at the tip of my penis, it did appear irritated. It does not feel painful/burn when I pee. However, at times I do feel slightly sensitive (I guess a slight tingling sensation inside the urinal tract around the penis head region) in that area at other times than during urination. I should point out that I was not drinking a lot of water for a period of days before I had sex. I have been drinking more water and cranberry juice since. The color of the penis tip has returned to a more natural pink, yet I still have slight irritation inside the tip.

Underneath of my penis head a small region of my skin was slightly red but it wasn't painful. In addition, I noticed a tiny white bump probably less than .25mm in diameter. Again, no pain when I touch it. I do masturbate a couple times a day, so I'm wondering if I have irritated it.

Skin in my "triangle area" feels itchy, but I see no irritation nor redness. I had trimmed my pubic hair about 3 weeks ago, but it wasn't shaved. Just trimmed with some small clippers. I did not cut myself when I did that.

Other things I'm noticing are a slightly sore throat, but I don't see any swollen lymph nodes, as I think those are in another region on my throat and wouldnt' those be seen on the outside?  Also there has been some cold weather.  Some testicle pain as well. My right testicle feels slightly sore and for a couple nights ago it was in pain. I should also point out that my father had to have testicular surgery because one of the testicles got twisted.  I don't remember the name of the condition but I recall hearing that it's genetic. I have felt this pain before in the past, before I had sex last week.

I must stress this is the only time I've had sex for a few years. I have been tested before this encounter and was clean as of those tests.

1. What are my chances of picking up any std? (Hepatitus, Herpes, HIV, any of the bacterial.
2. Could I just be having some Urinary Tract Infection?
3. Could the latex be the cause of the irritation of my urethra? The Masturbation?
4. I'm planning to go to Planned Parenthood after 15 days pass since I had that sexual encounter. Since they are a national chain, what are the specifics of the full STD scan they take there?
5. Is 2 weeks long enough to detect Hepatitus? What is my risk?
6. HIV - what's my risk? What's an effective time to test? I'll be testing at this 2 week mark, but should I calm down if I test am OK at the 6 week mark? 3 Months? 6 months?
7 Should I be worried?

Thank you

Let me reword this...

I should point out she was a sex worker, and she was pregant. I don't think that would make much difference in my risk, especially from what the doctor said.

Is that correct?

Johnny1509 - Considering I had paid for this answer, please do not hijack my thread.

Anyway, I wanted to give a status updated.  As said by everyone on here, I'm stressing about this minor incident too much.  I still have the irritation, but I went to a GP/IM today, and he told me pretty much the same.  I am at very near 0 risk, but he said for peace of mind, I should do the 3 month test and let it be after that.

I too have had some concerns.  First off, I suffer from General Anxiety Disorder which means I can take the slightest little thing and in my head turn it into some "marriage" ending event.
I am a 35y/o male, married for 10 years.  I have been faithful for the most part except for an event 2 years ago.  I was having an affair with a 19 y/o gal for 4 or 5 months.  Always wore a condom w/ vaginal and anal intercourse........I ended things 'cause my brother gave his wife "The Clap" and later admitted to an affair...so I became scared to death that this could happen to me.  I started checking my penis for the slightest sign of anything, trying to squeeze anything out of it, spreading my meatus looking for anything inside, stretching out my scrotum looking for any sign of anything, feeling my testicles many times a day, etc.  I got myself so worked up I irritated my meatus, made my testicles tender (not painful at all, just tender) Couldn't sleep at night, constant state of worry.  Went to local county health clinic. They drew blood and collected a urine sample.  Everything came back negative (chlamydia, ghonnorhea, syphillis, HIV, HSV-1, HSV-2, Hep A,B,C)
The results took a little over a week, and while waiting for that, spent nearly $400 on 28  100mg doxycycline tabs, 3  500mg Levaquin and 20 Flagyl tabs.  Had them overnighted. Placed myself on these without ever being told I needed them.  Took them as specified, results came back negative and all.....

Now here is the thing,  in Aug 2005, I received unprotected oral sex from a gal I met at a bar.  Now everytime my wife says her tummy hurts, or she has lower abdominal pain or lower back pain, I freak out all over again.  I know the cure for this is to STOP messing around.  Here again, I found a web site that does anonymous ghonorrhea/chlamydia screening via DNA replication in Urine.  Spent $149 on something that would have been free at clinic, but it says results in 2 business days.  I have worried myself so much that I might have picked up something from this oral sex and gave it to my wife.  Last week, we had hard vaginal sex and she had a tiny bit of blood tinging on toilet paper afterwards, but none since.  She has lower back stiffness, and occaisional lower abdominal crampy feeling. No discharge or odor, no more bleeding, no vaginal irritation, etc.  So I think I am freaking out over nothing.  My latest lab results won't be in until Monday so this will be a long weekend.  I have no discharge or visible sore, however, my meatus is a little red (probably from me checking so often) and my testicles are a little tender (probably from me checking so often as well)

How likely is it to get chlamydia or ghonnorhea from oral sex???
My County clinic says if I had something, I would deffinitely know it and that websites that say not everyone is symptomatic is misleading due to people not admitting to symptoms.

irritation has occured again in both urethra and skin below head.  I use Dove soap and/or Dial antibacterial body soap as well.  I'm just trying to figure out what could be the cause as well as trying to see if something trivial as the soap would be the cause.... =\

What do you think?

Dr. H may disagree with me and tell you the real deal, but the itching and redness could be a whole host of other things and not necessarily an STD, with the most simple explanation being a simple skin irritation.
If it puts your mind at ease, by all means get tested, but for what it's worth I doubt that you acquired HSV2 from this event based on what you described.

Thanks for the comments.  This has been the only encounter.

You can't get a STD through an intact condom, maybe aside from herpes or HPV - and even then condoms are really pretty effective. And your partner even gave you oral with a condom, which is more than most people would do. If you do have a STD, it's not from this encounter.

Thanks concernedinsf.  I now await for the doc's response.

concernedinsf - no scarring if that's what you mean.  No smell or anything to that effect.  That would have immediately turned me off.  It was well lit enough for me to see her region.

Speaking as an informed individual who has both types of Herpes, I would say that the symptoms you described, as a whole, are not consistent with a gential herpes infection.

Did you notice anything unusual about your partner's gential area during this act?

If you did test + for HSV2, I would be surprised if you contracted the virus from this act alone.

Followup question...If I do test positive for some bacterial infection...would that also mean if that if I got that type of infection, I would also have been open to get HIV or hepatitus?

To answer your question, it was totally covered the entire time.  From the just before first oral to the vaginal to the oral where I ejaculated, I kept the condom on the entire time.

if you had protected sex why are you testing in the first place, are you testing for unprotected oral sex ( getting a BJ).

Just a quick add on to my question:

"Underneath of my penis head a small region of my skin was slightly red but it wasn't painful. In addition, I noticed a tiny white bump probably less than .25mm in diameter. Again, no pain when I touch it. I do masturbate a couple times a day, so I'm wondering if I have irritated it."

The redness has gone down, but i still see slight difference in color between that area and the rest of my penis.

The several comments (below) that preceded this response are generally accurate. You describe a totally safe sexual encounter.  Since you wore a condom that remained intact, you can rest assured that whatever is causing your symptoms, it isn't an STD.  In any case, most of the symptoms don't sound much like an STD anyway.  Subtle color changes of the penile skin usually are meaningless, as is subtle testicular pain.  Much of it may be nothing more than anxiety over the event, magnifying normal body sensations.  To your specific questions:

1) Close to zero risk for any STD.

2) Non-STD UTI is rare in men under 40 years old.

3) Conceivably you could have a latex allergy, or chemical irritaiton due to lubricant in the condom, especially if it contained a spermicide such as nonoxynol-9.  Check the label on the condom packaging.

4) It would be silly to seek STD evaluation on the basis of the encounter you describe; a waste of time, energy, and money--both yours and Planned Parenthood's.  I can't say what testing any particular PP clinic might do; if they are smart, they will listen to your story and perhaps not test you for anything, unless you are at risk because of other sexual contacts you haven't mentioned.

5) Hepatitis risk zero from that exposure.  Anyway 2 weeks is too soon.

6) Zero risk for HIV, even if your partner was infected--which she probably wasn't.  I recommend against any HIV testing at all.  But if you need to do it for peace of mind, get a single test at least 6 weeks (preferably 3 months) after the event.

7) No.

Good luck--  HHH, MD
