worried very worried

about 16 days ago while in macau I had intercourse with a Vietnamese CSW  in the sauna of the hotel I was staying at in macau im a 22 year old male and realize now it was the stupidest thing I ever did. But since this was my first time having sex I don't know if I did anything extremely dangerous outside having intercourse with a CSW, The worker started by washing me with her breasts then after we moved on to have condom protected oral and vaginal intercourse,   so my questions are,

1) she gave me a BJ with hot water in her mouth before continuing to what I assume is the normal way then without changing the condom applied lubricant and went on to have intercourse. would the water have done anything to weaken the condom?
3) I have excema and before the sex I went to the sauna and hot tub  in the facility would this have done anything to increase the irritation of my skin and allow the HIV or STD to pass in more easily? as well as their may have been a small cut at the base of my penis does this put me at great risk as well?
4) After she got off me I accidentally touched the condom and got Lub and perhaps her fluids on my hand on my hand but did not wash immediately is this a significant risk as well? I don't believe I had any cuts on my hand.
5) after I finished I pulled it out everything looked in tact she then removed it from my penis with some paper towel and throw it out. I don't think anythings leaked when she removed it then I showered once in the brothel and once in my room.

im a student currently on exchange when I got back to my university I went to the clinic and had an STD screening done for herpies, HIV and etc how accurate would it be 10 days after exposer. I haven't been able to sleep for nights and I've developed a bad cough with yellow flem and random shakes it may just be my nerves but I really feel like im gonna die.... but i wanna know how much at risk am I to HIV and how soon should i get tested?

thanks doctor

Relax.  All your questions come down to your opening statement that "...we moved on to have condom protected oral and vaginal intercourse."  That the exposures were condom protected is the key point; all the rest makes no difference at all. If this your only STD/HIV risk, I would not have recommended screening at all.

Note that this site is limited to STD issues, not HIV. But my responses apply to HIV as well as STDs.

There is no reason to believe that having water of any temperature in the mouth could make a difference in STD risk, even without a condom and certainly not with one.  Eczema or any inflammation theoretically increases the risk of herpes or HIV if exposed, but you can't increase zero risk to a meaningful level, so it has no bearing on your situation.  You can't catch any STD by hand contact with genital secretions, even if you immediately then touch your own genitals. How you wiped up afterward is irrelevant.

Your cough and other symptoms cannot be due to HIV or any other STD from this event. You caught a cold and you are anxious, that's all.

I hope this helps. Best wishes--  HHH, MD

You can't come back for every anxiety-provoked additional "what if" or "yes but" question that enters your mind. I saw your comments about the pubic hair in your original question. If it was a concern, I would have said so.

I don't make psychiatric diagnoses from a distance. "Paranoid" is your term.  But clearly you are irrationally focused on something that simply isn't worth worrying about.

Hi doc,

im sorry I just have one quick question, I got my STD test back today and I'm clean of everything( except HIV technically because of the window) but I was talking with the doctor and he said since the condom was on their is no problem he just mentioned that as long as as I didn't come into contact with any other fluid. Im just worried now becuse she was nude when she bathed me and I may have had some sores around my butt and other parts of my body could her saliva or anything have entered during this period to transfer HIV? The only reason im asking is because i have a date soon and I could not live with myself if things progressed and I infected her. Am I just being paranoid???

Hi Thanks again for helping to calm my nerves i dunno guess I was getting paranoid that the condom broke or when she gave me oral it may have made holes in the latex actually at one point  I was afraid it was a bad condom because she provided it. But if it broke I'm sure she would have known and so would I and even if she sucked on my testicles during the oral the risk is almost zero.  Ok so now I can relax thanks allot  doctor ill still get checked in 3 months just to calm my nerves finally it's a great thing you got here and what your doing when I went to talk to the doctor here he gave me a religious speech which bothered me. But anyway I know from other posts you don't like paranoia so ill see my doctor Wensday as planned and do a  follow up for myself and I thank you so much please take care.
