Just in case!!

Hello Doc,

Just a quick one!...I had 2 unsafe exposures.  The first one was in China and it was unprotected oral from a women.  The second was also in China and was also unprotected oral but as I was drinking I remember putting on a condom and inserting into her vagina for a very short while but after I was concerned that the condom fell of, even though I think I remember taking it off after the fact but that is worse case secerio.  This happened in Sept of 2008 and below is the testing I have had.
- HIV at 7 weeks, 15 weeks and 9 months

- STD test for Clymidia and gonnerea at about 8 months after (this was a urine test)  I was concerned cause I peed about 1 hour before testing

Since these exposure I have been on Biaxin for 10 days at 500mg twice a day, Amoxicillian 500mg 3 times a day ( I have been on this twice as I have a saliva gland issue for a number of years.  I was also on Cipro 500 twice a day (I think) for a sinus infection.

All test were negative.  Also I have had no symptoms whatsoever.  Below are my questions and concerns

1)  I never got tested for syphlis after these exposures.  Could I have it even though the meds I have been on and also with no symptoms?.

2)  As I have never had any other symptoms of STDS is it still possible to have gonnerea or clymidia?...I was just concerned as I peed 1 hour before the test.

I figured you didn't take them together.  Either drug alone would cure syphilis.

Thanks for the answer...just one last point,  the biaxin and amoxicilian were taken at different times around 2-3 months apart.  Biaxin was for a Resp infection and the amoxicilian was for my saliva gland issue.  These all have been taken throughtout the last year since exposure.  Would Biaxin or amoxicial cure syphlis by themselves?.  no more questions from me so I would like to thank you for your time!.

Wow!  You were grossly overtested in relation to the exposures you had.  But in any case, it is clear you didn't catch anything.

1) Syphilis was unlikely, and almost always causes symptoms (a sore on the exposed part, i.e. your penis -- which you would not have missed).  But in any case, if you caught it the clarithromycin (Biaxin) and amoxicillin would have cured it.

2) No.  Chlamdyia was no risk at all (it isn't acquired by oral sex and the vaginal was protected) and gonorrhea, which can be acquired by oral sex, almost always causes symptoms and in any case would have been cured by the antibiotics you have taken.

You didn't catch anything.  You are far more worried about that exposure than most people would be, and there is no basis for such concern.  Time to let it go.

Good luck-- HHH, MD
