Sometimes signs such as lymph node swelling or even redness (which as you point out is highly subjective and subject to over interpretation) can persist beyond the completion of therapy. I would not worry.

one last question if you dont mind, i have 2 days of my antibiotic treatment left, it seems to have knocked most of my symtoms out except for my tender lymphnodes and some irritation and readness at the tip of and maybe slightly inside of my ureathra, question is it safe to resume unprotected sex with my wife, other question, i have read various threads where people seem to not be able to kick the redness around the tip of there penis i am kinda nervous that this will happen to me, prob. adding to the problem haha. anyway to the point, at the end of my treatment if my situation is not fully resolved what would you reccomend that i re test for or re treat with. i am egar to move on from this thanks doc!

Syphilis is nto a conern. Even if you were exposed, the antibiotics you took would have taken care of it. Hepatitis A and C are not transmitted among heterosexuals and there are no cases of hepatitis C being transmitted by oral sex. You need not be conerned. EWH

I am very hopeful that the doxy and zithro will clear me up, my last question for you is what is the likelyhood that I caught another STD from this encounter that I was not tested for ie. Syphillis or heb a,b,c? With everything going on I feel like I could have missed any small lesion from syphilis, has it been long enough to take a conclusive test? Do you recommend? Also 6 week neg for HIV is conclusive in this case? Thank you
Welcome to the Forum. I'll make some comments but the problems that you outline will probably need to be followed, preferrably by the same clinician, to decisively sort things out. Your symptoms soon after your exposure were most likely due to irritation by the Purell. Symptoms due to STDs do not begin within hours of exposure. On the other hand, chemical irritation typically resolves over a few days. Interpretation of your situation is further complicated by your self medication which could be causing an allergic reaction manifest, at least in part by the redness you noticed on your penis.
Att his point I would do the following:
1. Continue the doxycycline that you were given, along with the dose of azithromycin. This will cure bacterial NGU is this is contributing to your problems and likely cure epididymitis if present (even when taken for 7 days since it was given with the azithromycin).
2. If your symtoms have not completely resolved (I suspect they will have) return for evaluation by your doctor before you complete the doxy. A repeat examinination should help to clarify things for you.
I would not worry about permanent damage.
