NGU concern

Dear HHH or EH
Thanks so much for taking my question. i am a 26 year old male who has only had 1 sex partner in 6 years except for a breif oral encounter about 8 weeks ago. The breif oral encounter in which i was the insertive partner is the encounter in question. right after the encounter i freeked out and used purell on my genitals including the opening of my ureathra, i was very libral on the amount i used also. i realize now that this protected me from nothing i tested for stds at 2 weeks and 7 weeks, neg for hiv, gon, clam. positive for hsv1 and low level hsv2. i have never had painfull sores on my genitals and i never get cold sores. about 3 hours after i used the purell on my self i devloped pain after urination and it never left, after 3 weeks the symtoms got worse and i devolped constant itching around my testicles and penis, and eventually my testicles got to be sore and tender,lower lymphnodes tenderness, also had readness at the end of my penis and i noticed one time a small amount of milky discharge in the am...so at week 7 i went to my GP she perscribed me 2 tablets of zithro and 7 day twice a day doxy for NGU or ureathritius. she also advised me to discontinue cipro which i had been self treating with, my questions are could the purell caused this infection, and if so will the NGU treatment cure me? i did not tell my GP about the purell. also with reguards to my testicular symtoms do i need to be concerned about Epididymitis? in your opnion do you think that i got this from the oral or are my wounds self inflicted. i am concerned that if i have Epididymitis that the 7 day dosage of the doxy will cut off short as the reccommended dosage by the CDC is 10 days. its been almost two months and i just want to get better and put this behiend me, i am 2 days into my zithro and doxy treatment and i think its working, just hard to tell as i have a cold and some sinus activity aswell. lastly do i need to be worried about any permenant damage?

Sometimes signs such as lymph node swelling  or even redness (which as you point out is highly subjective and subject to over interpretation) can persist beyond the completion of therapy. I would not worry.

one last question if you dont mind, i have 2 days of my antibiotic treatment left, it seems to have knocked most of my symtoms out except for my tender lymphnodes and some irritation and readness at the tip of and maybe slightly inside of my ureathra, question is it safe to resume unprotected sex with my wife, other question, i have read various threads where people seem to not be able to kick the redness around the tip of there penis i am kinda nervous that this will happen to me, prob. adding to the problem haha. anyway to the point, at the end of my treatment if my situation is not fully resolved what would you reccomend that i re test for or re treat with. i am egar to move on from this thanks doc!

Syphilis is nto a conern. Even if you were exposed, the antibiotics you took would have taken care of it.  Hepatitis A and C are not transmitted among heterosexuals and there are no cases of hepatitis C being transmitted by oral sex.  You need not be conerned.  EWH

I am very hopeful that the doxy and zithro will clear me up, my last question for you is what is the likelyhood that I caught another STD from this encounter that I was not tested for ie. Syphillis or heb a,b,c? With everything going on I feel like I could have missed any small lesion from syphilis, has it been long enough to take a conclusive test? Do you recommend? Also 6 week neg for HIV is conclusive in this case? Thank you

Welcome to the Forum.  I'll make some comments but the problems that you outline will probably need to be followed, preferrably by the same clinician, to decisively sort things out.  Your symptoms soon after your exposure were most likely due to irritation by the Purell.  Symptoms due to STDs do not begin within hours of exposure.  On the other hand, chemical irritation typically resolves over a few days.  Interpretation of your situation is further complicated by your self medication which could be causing an allergic reaction manifest, at least in part by the redness you noticed on your penis.  

Att his point I would do the following:
1.  Continue the doxycycline that you were given, along with the dose of azithromycin.  This will cure bacterial NGU is this is contributing to your problems and likely cure epididymitis if present (even when taken for 7 days since it was given with the azithromycin).

2.  If your symtoms have not completely resolved (I suspect they will have) return for evaluation by your doctor before you complete the doxy.  A repeat examinination should help to clarify things for you.  

I would not worry about permanent damage.
