Condome Broke

Dear Doctor,
on the 12th of  February I had a protected sex with a russian sex worker in Dubai, in one of the 3 intercourses the condome was broke in intercourse lasted for arounf 8 seconds, my penis was not completely in her, and I did not feel that it was wet, the exposure was not intense at all. I did STD test for clamydia and other STD on the next day they turned out negative. after 14 days from the exposure, during which I had no symptoms, I did HIV 1 and 2 test they turned out negative? what is the reliability of such test in the light of the nature of my exposure and without the presence of any symtoms. the anixiety is killing me, please i need your opinion as soon as you can.

Your risk is low.  Even though the condom broke, your exposure was brief.  In addition chances are your partner was not infected.  At two weeks about half of HIV tests that are going to be positive will be however, in you case, the odds are very much in your favor.  I would not worry.  It is unlikely that you acquired HIV.  Get another test at six weeks to convince yourself, it will be negative, then move on.  No further testing is needed after your 6 week test is negative.  EWH

Doctor i was reading another thread of some one who had unprotected sex with massage parlor lasted for 3 minutes, and eventhough you told him that he does not need to be tested at all as his odds are very low, while you c that i still need a six week test eventhough my exposure was shorter from his.

Theoretically the addition of the p24 Antigen test in the Duo test should allow increased sensitivity for detection of infection in the early stages.  Precisely how much I cannot say.  Additional tests such as PCR testing and P24 antigen tests are not routinely recommended for diagnostic purposes here in the U.S. and I have little information about what the added tests do in terms of detection.  I do know however that they are more susceptible to false positive results than the standard antibody tests - this is part of the reason that they are not recommended.   In my judgement (which is what you ask for) you still need a six week tests.  EWH

But how about the reliability of the Duo test that I have done, as my provider said it's conclusive at that time and no further testing is needed.

From a statistical perspective, a three week test still might miss about 1/3 of infections.  You risk is low however and the test result you mention makes your risk even lower.  As I said before, you woul have a higher degree of confidence after a six week test.  EWH

thanks for your reply, i got P24 Duo combo test 21 days post exposure, it was negative. do I need further testing? am still worried
