Worried I got more than I paid for

I will start by saying there was no vaginal or anal penetration whatsoever. I went to an escort for a hand job with some seduction but it went further than anticipated. There was some unprotected very brief oral sex that amounted to licking and sucking the very tip of my penis She also straddled me and rubbed her vagina against my penis, but kept her panties on so there was a material albeit very little material between direct genital contact. She then gave an intense hand job which was rigorous enough to break the skin in a few areas below and on the under side the head of my penis (circumcised) on the layer of skin that separates the head of the penis and the shaft, the fleshy little section in between. It wasnt enough to draw blood but it was definitely broken skin. She used some kind of massage lubricant and also drooled saliva for extra lubrication during the act. I worry that any vaginal secretion she may have had came through the panties and was on my skin as well as the saliva from her mouth could have entered my bloodstream because of the abrasions and at the tip of my penis with her hand and the lubricant spreading all of this all over my penis.

This information does not change my opinions or the advice above.

1)  There is no need for testing.  However, since you're obviously being tested anyway:  that result is probably about 50% reliable.  The oral fluids test requires at least 6 weeks and maybe 3 months to reach 100% reliability.  If you want conclusive results soon, visit a clinic or doctor for a duo test (for both HIV antibody and p24 antigen) 4 weeks after the sexual event.

2) Regardless of the vigor of the masturbation, no HIV infections have been known to be transmitted by hand-genital contact.

3) A previous customer's sexual secretions would be too diluted to transmit infection in this way.

4) There is no risk for hepatitis or other STDs.

That will be my last comment. Do your best to accept the scientific evidence and the reasoned reassurance you have had and move on with your life.

Dr. Hunter,

It's been about a month since I've reached out. I initially felt better after your post and tried to resume some normalcy. I've had one sexual encounter with my girlfriend in which she gave me oral sex to completion and swallowed. We did not have any other type of sex. I'm now worried sick about both her health, our unborn child's, and my own. I have had stomach pain/slight nausea throughout this month, but that has intensified after about 20 days since the encounter with the sex worker. I feel nauseous to the point that I may get sick. My lymph nodes in my neck have definitely been impacted as I can see my pulse in my neck due to throbbing. I have pain underneath my jaw and by my collar bone. All of this seems to be signs if acute hiv infection. I took an oraquick home test at 20 days and it came back negative. I have read a ton and I realize this can be paranoia and stress, but can't seem to ignore the undeniable physical symptoms I am experiencing. So here it goes:
1 - how reliable is an oraquick test 20 days after exposure and would you suggest a re test
2 - I understand that from reading there hasn't ever been a known case of hiv transmission receiving oral sex, however as per my initial post, I was more concerned about the rigorous hand job that broke the skin and the saliva that was used as lubricant.
3 - is there any risk of hiv if the sex worker had done anything unprotected with an hiv positive person before me? Like remnants of semen by the gums or something like that?
4 - would other diseases such as hepatitis be a real concern here with this type of encounter and produce similar symptoms to what I am experiencing now?

I realize you get a lot of frustratingly paranoid people here and I am sorry to be part of that group, but I can't feel normal with these symptoms and need to understand what I potentially am at risk for.

Thank you

Nope.  My bet is that your remaining symptoms are attributable to anxiety, nothing more.

I got the test results back and as you predicted they came back negative. The bumps i mentioned seem to be fading. I have still had the urination frequency and urge, and have had some stomach/Lower abdomen pain intermittently. Anything to worry about as far as stds or HIV are concerned?

Welcome to the forum.  Thanks for your question.

Stopping for an initial comment after seeing the opening sentence of your question:  with neither vaginal nor anal penetration, you are at very low risk for any and all STDs.

Now having read the rest, the entire exposure was low risk.  The brief oral exposure was unlikely to result in any infection, and hand-genital contact is virtually risk-free even if vaginal secretions or saliva are used as lubricant. And no STD is likely to be transmitted though clothing.  Your symptoms are not typical for any STD, but urinary urgency and frequency suggest genitally focused anxiety causing increased bladder tension.  As you seem to understand, the main symptoms of a urethral STD like gonorrhea, NGU, or chlamydia is abnormal urethral discharge.  "Small slightly raised painless red dots" in an area of abrasion also are not alarming.

So you can definitely expect negative results on the gonorrhea and chlamydia tests.  It was a little early for chlamydia, but fine for gonorrhea -- but in any case, there is no realistic risk of either infection from the exposure you have described.

So I see no need for any additional STD testing and I expect your nonspecific symptoms to clear up on their own.  Of course if they do not, you should be professionally evaluated, but in the meantime I wouldn't be at all worried.  If I were in your situation, I wouldn't hesitate to continue unprotected sex with my wife (pregnant or not) without fear of infecting her.

I hope this has helped.  Best wishes--  HHH, MD

Symptoms: I feel like I am urinating more frequently and after urinating have a strange feeling like I could urinate again.
- I noticed two very small slightly raised painless red dots on the under side of my penis in the same fleshy section where the abrasions occurred. they are on separate sides of the under side of my penis (left vs right). They are isolated with no additional marks or clusters from what I can see with the naked eye.
-  no discharge or burning when urinating.
- got a ghonnorea and clamydia (sp) urine test a little over 48hrs after the incident but no additional testing.
- want to know how risky this incident was, what stds I could rule out, if there is any risk of HIV. Would you recommend additional testing and if urine test would be accurate after 48hrs? I have a pregnant girlfriend but there's been no sexual contact whatsoever since the incident.
