Oral

Dr HHH,

After having read some postings, I think I know what your answer will be, but for peace of mind, I just want my specific situation addressed.  A couple weeks ago, I received oral from a prostitute for about 2 minutes.  I was asymptomatic until about 6 days ago.  I felt pressure from the prostate area.  One visit to the Dr yielded a shrugging of shoulders (I didn't not share the sex part with him).  A finger found no prostate abmornality.  The following day, fever and chills accompanied an increased pressure in that area.
The second visit and exam led to a diagnosis of prostatitis.  Urinalysis - neg (no urethral swab).  I was given 10 days worth of Cipro.  I was feeling OK for a couple days, but the pressure comes and goes.  I would think that, after four days of antibiotics, the symptoms would have disappeared, assuming there was a bacterial infection (should that have shown up in the urinalysis?).

So, I have an appt with a urologist tomorrow.  Should I bother being tested for an STD?  I don't want to pass anything on to my girlfriend.  STD or not, this scared the **** out of me and tuaght me a lesson.  I feel awful about it...

How in the world could you be "more concerned" after my reassuring comments??  I told you that an STD doesn't explain your symptoms.  Deal with the prostate problem and mellow out about STDs.

In general, oral sex is safe sex, but that term doesn't necessarily mean zero risk; technically a better term is "safer" sex.  The risks from receiving a BJ are for gonorrhea, herpes, and nongonococcal urethritis (NGU), but all those risks are low.  The risks are zero or close to it for HIV, chlamydia, HPV, and hepatitis B.

In the absence of symptoms you can be confident you didn't catch anything.

So, is oral sex not truly "safe sex"?  What STD is even reasonably likely to be transmitted (herpres, clap, other)?  I have no symptoms at all for those.  Should I assume at this point I don't have them, based on my lack of symptoms?  I've read your other postings where you say not to worry at all about STDs from oral.  Now I'm a bit more concerned...

Most cases of prostatitis, or chronic pelvic pain syndrome in men, probably do not represent true infection of the prostate gland.  However, the occurrence of fever, chills, and increased pain ("pressure") after a digital rectal exam suggest that you might have had true bacterial prostatitis.  That condition explans maybe 10% of prostate problems in men.  Cipro is a commonly used antibiotic for it.  It's not an STD, and the whole business probably has nothing at all to do with your adventure with a commercial sex worker; and you probably can be confident you have nothing you can transmit to a sex partner.  Of course I cannot guarantee you don't have additional issues, such as some other STD in addition to the prostate problem.

Discuss these issues frankly with your urologist and follow his or her advice.  But I don't think you have anything serious to worry about.
