Panicing: insertive oral w/possible HIV+

I had my first (and last) encounter w/a guy one week ago today - it was unprotected insertive oral only. He said beforehand he was clean, but then afterward when I questioned him again (because he looked liked the picture of death after I saw him w/his shirt off - skinny, an untreated hernia, coughing, what looked like small sores) he said "well, it's nothing you have to worry about." I was floored. He has ceased contact with me. The thing tuned me off so much, that after 15mins I had him stop and I masterbated to completion (which also took a while) - which bothers me because I feel like I could have been grinding in a virus into my urethra, etc. I showered and urinated as best I could directly after. I have not been w/my wife since.
I had UTI-type symptoms (constant burning/tingling toward end of penis, urgency/frequency, but no discomfort urinating) and pressure in perineal area after 3 days and had an IM shot and I'm 4/7 days into oral doxyl. with little improvement - still have constant but non-actute burning/pain, and some urgency.)
I have a sore throat and some headache, but so far no temp (I'm usually about 97.5 degrees, and I haven't had a temp over 99 since I was a child) and no nasty rashes (though it's amazing what you can find when you look...) or swolen glands.  The constant (though low-grade) pain and tingling and urgency, the sore throat, and every skin mark and small body change is making me panic, and I'm self-medicating with 5mg (10 year old vintage) Valium 1x or 2x a day, and some Valtrex just for the heck of it. I overcame my embarassment & took a PCR test yesterday at six days. Acording to the web, oral is considered low risk, and I understand my sore thoat and other things can be anxiety-driven.  I'm having a hard time waiting out the few days to the PCR test results, and I'm not sure if 6 days is enough time to show results - if the test isn't ready by the weekend I'm not sure how I'll cope. Not sure what my question is...

The DNA does not replicate further once it is in the tube.  The conditions are not right for the virus to continue to grow.  

Your experience is part of the reason we do not recommend on-line services. They also typically do not provide you with the possibiltiy to discuss your test results and their interpretation with a trained professional.  EWH

Your comment is confusing to me.  The highest estimate of risk is 1 in 10,000 and as I said I beleive it is less that that, i.e 1 in 100,000 or even lower.  EWH

Dr. from reading your above comment did you mean to say that catching HIV through unprotected oral is too low in your opinion?  You said 1 in 10,000 was too high.  Just trying to clarify for myself.  You guys do a great job on this site helping people!

DNA test from +6 day sample is neg., but still worried about symptoms, even if they alone are not enuf.  I'm having trouble finding percentage effective rates for such early DNA tests, but I saw nothing earlier than 7 days mentioned. I suppose to put my mind at east I'll take to take another now that I'm at +14 days and I think I should be covered, no?

I don't have a regular GP at the moment, and I didn't relish going to some public STD clinic - and my city/county HIV info line was quite unhelpful (surprising seeing it's a major city) - they didn't offer a DNA test, and couldn't even point me to another resource. (I suppose if it were an occupational exposure, it would have been done by the person's peers - otherwise the test is probably not popular as it's expensive.)  I'm surprised because although my exposure (if any) was supposedly low-risk, *they didn't know that*, and if I'm asking for a paid DNA test where PEP or general early treatment could be benefical, I'd think for the general welfare they'd want to provide a test and have a quick turn-around.

[FWIW, this on-line service does include consultation with an MD (for any positive results) for all but HSV-1.]

Thje sample was taken at only +6 days, so that may be too early anyway, so I won't feel really good about it until a 2nd DNA test in a few weeks or an antibody test in a few months.  

Question - in case it hasn't been asked already -- what happens to virus (assuming some other sample that contains some :))  ex-vitro in the sample tubes as they're sitting there for a few days? The DNA traces are still there, but will the virus continue to multiply if temps are not extreme? - I'm only asking for acedemic purposes.

The test was ordered from one of those online sites - I wasn't sure what their relationship was with the lab or what tests were done where - guess I'll know to ask next time - oh, wait - there isn't going to be a next time :)

BTW - the last msg was only meant to be constructive - I do think that you are providing a valuable service.  I can be told the odds, but we are emotional creatures after all - and "the man" has done a good job of making us paranoid regarding HIV transmission. (As Eddie Murphy said, "now you just put it in, and your **** explodes...")  It doesn't help that stress, antibiotics and other things can make you feel like crap, and that there's no smoking symptomatic gun, and early-detection tests and instant test for potential partners are not readily available and cheap OTC.  What we need is an all-STD dipstick...

[Regarding being on the wrong forum - an HIV search on Google got me to this forum so I posted here - I think you should have (or get) the ability to move posts between forums to the correct one so that this can be corrected, and not blame the user, especially if there's a charge for the service, and considering he/she may be a little stressed...]

The @#$# PCR test still isn't ready - it was sent x-country to another lab rather than being done locally - if I'd known that I would have tested somewhere else. (I don't think people appreciate how fast people need these results...)
Day 9 and sore throat is worse, but still no temp or swollen nodes - but I do have white patches on my tongue - I understand this can be from the Doxycycline (as well as mimicking other ARS-like symptoms), so I'm stopping the antibiotics, which is probably okay seeing the urine screen didn't show anything. Still freakin'...

Sorry, I neglected to also remind you that you do not know that your partner had HIV, Thus there is a high probability that you were not even exposed to HIV.  Sorry to have left that out.  EWH

You have a better chance of winning the lottery than catching HIV through oral sex!  Anxiety is a very powerful thing.  It's a good thing to get checked out.  Trust me when I say your level of risk was VERY VERY LOW.  Just be careful in the future and get to know your partners better.  I really wouldn't worry about this at all.  I believe all of your symptoms are from anxiety.  Also I wouldn't be taking medicine that is 10 years old.  Let your doctor know your stress level and they should give you something to call you down.  I would suggest Xanax because it does the trick and works within 30 minutes.  Xanax can be addicting though so just be careful.  You will be ok my friend!

Sounds like not the best of choices and I understand your concerns.  Fortunately, the quoted figure for HIV risk, if one has oral sex with an infected partner is less than 1 in 10,000 and, in my estimation that is too high. Some experts state there is no risk at all from oral sex.  Neither of us on this site have ever seen or reading the medical literature of a convincing instance in which HIV was passed by oral sex.  This includes by all of the people who appear to be in ill health such as your partner.  Your risk for HIV is virtually zero.

As for your symptoms, I am confident that they are not related to HIV. The onset of the symptoms you have experienced is too soon for the ARS which typically comes on at 2-3 weeks following exposure, not at one week and which are far more, and more severe symptoms than you have described.  this is some other process, perhaps even anxiety.

Your PCR test should be back soon. The test itself only takes a few hours to run in the lab.  A negative PCR will provide conformation that your symptoms are not due to HIV as person with the ARS are uniformly viremic and have positive PCR tests.  At the same time, to be completely honest, if you had acquired HIV (I am confident you did not), the time course of PCR positivity is still not completely worked out and you will still need antibody testing to be completely sure with the results of an 8 week test being definitive.  I predict that such a test will be negative.

I hope these comments are helpful to you.  Try to relax, you really are not at risk from the exposure you describe.  EWH
