Please Advise

Hey Doc,

Here is the situation.

About 11 days ago I have protected vaginal and unprotected oral with a worker. The next day I notice dry skin below the head of my penis. That night I started to feel a burning sensation in my penis. I did not have any discharge. I went to the doctor and she look at the dry skin which had began to heal and she said there was nothing to worry about and it probably could be from condom irritation, she prescribed nystatin. The burning continued in the penis, not on the dry skin and the dry skin turned into a somewhat rash, it does not have any lesions and is not tendor, it just has some discoloration and wrinkles that I am concerned about. I went to a second doctor and he told me that I might have a STD (Chlamydia or gonnarhea) due to the burning sensation(Not while urinating). He prescribed me an azithromycin 1g. I had him take a look at the rash or whatever it is and he said that it is in the are where most herpes outbreaks occur but it does not look like herpes and since I didnt have any lesions that should not be concerned at all and that it could probably be from friction.

I guess the question is the dry skin that I developed the next day is it signs of herpes and could it be possible to have genitial fungus.have read post stating that is a possibility. But I guess the doctors would have tested me for herpes if they felt it was a threat correct?

As far as chlamydia, wouldnt I have had discharge and burning sensation while urinating if it was an STD. I am waiting for the test results, but I figured it would just inflammation of the urethra, being that I have had it before and no discharge was present just burning sensation in the penis.

One day is pretty quick; usually it takes 2-3 days for symptoms of urethral STDs to improve with azithromycin.  It might be it was going to clear up anyway.

For response and great answer. I took the meds yesterday and burning has cleared up. So does that mean I possible caught Chlamydia. Or something else bacterial related

I meant to add that I agree with your analysis of chlamydia, i.e. your symptoms don't really fit and neither does condom use -- and chlamydia is rarely if ever caught by oral sex.  And if that were the cause of your symptoms, they would have promptly cleared up following azithromycin treatment.  I expect your gonorrhea and chlamydia tests to be negative.

Welcome to the forum.

You had accurate replies to the same question on the herpes community forum.  Herpes does not cause the sort of symptoms you describe, nor does it mimic dry skin -- so I agree with your first doctor when she told you she did not suspect herpes (as you said on the community forum).  As for burning "in" the penis, it is true that chlamydia or gonorrhea could cause such a symptom, but rarely; the large majority of urethral STDs like gonorrhea, chlamydia, or nongonococcal urethritis (NGU) cause discharge as the main symptom.  And having used a condom, you were at low risk for any STD; oral sex is low risk even without a condom.  Finally, neither herpes nor any other STD can start to cause "dry skin" or other irritation the next day after exposure; 2-3 days is the minimum.  And if it was herpes, it would have progressed to open sores by now, or would have cleared up entirely.

For the reasons described, I am confident you do not have genital herpes.  As for other causes, the doctors who examined you are in a better position than I am to judge possible causes of the penile rash/dryness.  A fungal infection and virtually any cause of dermatitis are possible; most of the innumerable causes of skin rash anywhere sometimes involve the genitals.  An allergy to latex or to condom lubricant is one possibility, but that's only a guess.

If either the rash or discomfort in your penis persist, return to your doctor(s); if s/he isn't able to sort it out, a dermatologist may be a logical next step.  But in the meantime I really wouldn't be at all worried about herpes or other STD.
