Possible infection

Dear doctor,

I had protected sex with an escort about two weeks ago. Nothing was done unprotected. However, about 4 days later I started feeling a burning and itching sensation on the inside and tip of my penis. I immediately went to a urologist who told me that he doesnt think its an STD and I have nothing to worry about, he only prescribed Tavanic for me. I told him that I have a partner (the escort was a stupid thing) and asked whether I should refrain from having sex with my partner. He said no, as long as I take one of the Tavanics, we can proceed as usual. Problem is the feeling is still there. there is no discharge, but the penis is a bit tacky. I have also had chlamydia before, same symptoms, and I contracted it although I used a condom. I have now gone to another doctor whose view is also that it is likely not an STD, but maybe inflammation due to the condom. He also gave me Diflucan. I have in the past eperienced a reactino to condoms, but that was almost immediately, not four days after sex. How can the allergic reaction / inflammation start so late?

Do I have anything to worry about? As I said I contracted chlamydia before even with the use of a condom and I have had a reaction to condoms before, but not 4 days after, sounds more akin to an STD to me. What should I do next?

Welcome back to the STD forum.  You had a similar question on the STD community forum last year, with a "wet sensation" as your main symptom at that time.  As I recall from a brief glance, I believe you also had had condom-protected sex and were evaluated for STDs with negative results.

No distant online source can give as good an answer as evaluation by a knowledgeable clinician after direct examination.  It would have been good for you to have chlamydia and gonorrhea testing before you were treated with levofloxacin (Tavanic) -- maybe that was done, but you don't say so.  Had one of those been positive, however, it would have had to come from someone other than the escort, given that sex was condom-protected.

A good bet for symptoms like yours, in the absence of STD, is physical irritation or allergy to something associated with the condom.  Spermicides can do that for sure, but certain other lubricants or latex itself could be reponsible.  The lack of response to levofloxacin, a broad spectrum antibiotic, is additional evidence that no STD or other bacterial infection explains your symptoms; chlamydia would have been cured if you had it.

I suggest you continue to follow up with the urologist.  You can be sure no STD or any other infection acquired from the escort.

I agree 4 days is a bit late for that explanation.  But I have nothing more to say.  This is something that's going to have to be worked out with your personal health care provider.

I forgot to add, I also used a wet wipe to clean my penis (also the tip) thoroughly after taking the condom off. This could have added to the irritation / allergy / inflammation maybe?

Hi Doctor,

That sounds reassuring. The itchiness is almost gone today, hope it stays that way.

I was just wondering that if it is a reaction to latex / spermicides / lubricants, how it could start so late. My doctor said its possibly a delayed inflammation. Is it possible for inflammation in the penis to be delayed for four days?
