Problems in the Genital Area

12-17-07 sex w/ mistresscondom came off.Vaginal, oral, anal.1-1-08 unpr.vaginal sex w/ wife been separated 1-15-08 sex w/ mistress unpr. vaginal, oral, anal.1-19-08,1-27-08 & 1-29-08 sex again Some time in Jan.son got the flu and I followed with shakes, body aches.1-30-08 tingling, burning, itching right side of scrotum, minor shooting pain head of penis.Pain in legs, ache lower back.On 2-8-08, .one side of the lip or urethra appeared swollen.Clinic Dr. Around 2-2-08 and saw nothing that represented an STD.Labs for HIV, Clamydia, Gorrorea were neg.Said if Herpes would be too early to tell and no lesions to swab. Then went 2 times to best friend Nephrologist. He said I was fine, but gave me 1000 mg Valrex 2 times per day in case.Took it for 10 days.Went to old family Dr. has seen thousands of STD cases.Said genitals perfectly normal including urethra. On 2-15-08 small area about size of quarter on left side of penis at base had shedded skin white in color.Shedding stopped later became small light red rash no pain whatsoever.patchlike, not painful.Since, scrotum tingling itching on right especially when I sit down, doesn't always last. w/ me about 30% of the time.If Herpes, never had anything that really represented a classic lesion and never had pain, liquid or puss.  At one point small amount of discharge from Urethra clear in color that could have been precum but wasn't aroused at the time. No real pain though. Just a lingering tingling itchy scrotum at times.Mistress lab work:

It was all there in the original message.  Her results are negative.  Technically borderline, but negative for all practical purposes.  She doesn't have HSV-2.

Your symptoms don't suggest herpes, which seems to be your primary concern.  The red flaky rash also doesn't sound like any other STD either.  Perhaps most important, you have seen at least a couple of health care providers who gave you pretty strong reassurance, you have been tested pretty extensively, and treated for the common infections.  My conclusion is that you can put all STDs, including herpes, aside.  I don't think any of your symptoms have anything to do with either your wife or your mistress or your sexual exposures to them.  Everr male gets an itchy scrotum from time to time, usually not a sign of anything wrong.

Most "light red" penile rashes and/or flaking skin are not STDs.  Anything from eczema to psoriasis to fungal infections (e.g., lichen planus?) is possible and I really can't speculate.  You might consider seeing a dermatologist, but otherwise I have no advice.  In the meantime, this doesn't sound like anything that could ever harm you or a current or future sex partner.

Good luck--  HHH, MD
