Prostatitis, NGU, or Herpetic Urethritis

Doc,

I am a 27 y/o male stuck on a ship, away from proper medical care.  So i will start from the beginning.  Last February I had several encounters with the same partner, unprotected.  In early april, i began feeling symptoms of pain in my urethra, cloudy urine, and split stream, although no discharge.  Unavoidably, i could not make it to the clinic until a week after the symptoms started.  By that point i was having problems sleeping.  I was diagnosed with chlamydia, given azythromicin, and a few days later things were on the upswing.  A month later, same type of symptoms.  They didnt believe me that i didnt get reinfected, Azythromicin and on my way, out to sea (where i work).  I then was given 10 days of doxy.  no true relief, more symptoms like the first, but also pain into the pubis/pelvic region.  Then a couple weeks later, another round of Doxy, then Metrondinazole and azythromicin.  I was then in port, got a digital rectal exam, (very painful) and was prescribed Avelox.  I took it for 35 days, and things were well, more or less.  Back in port, went to resupply on avelox, they didnt have it, doc put me on cipro/trinidazole combo.  Now a week later and the pain is back in my urethra.  

Could this be episodes of Herpetic urethritis?  I think the avelox worked, but perhaps it was just a good month?  The doc in port thought prostatitis (but he was also DDx-ing with wikipedia, i dont know if i trust it)

Welcome to the Forum.  I will try to help although your limited access to medical care at this time limits things a bit.  There are many elements of your history that are a bit atypical for chlamydial infection and are very out of line with what would be expected to occur with herpes.  

Are you sure than you had chlamydia?  Was a test performed?  Chlamydial infections are rarely severely painful and chlamydia is not a meaningful cause of prostate tenderness.  On the other hand, your symptoms are highly suggestive of prostatitis which is not an STI but is associated with all of the symptoms you describe.  Further, prostatitis typically does not completely respond to short courses of antibiotics of the sort you describe and often would require a month or so of antibiotics (and sometimes more than one course of treatment) to address the problem.

As for herpes- there is nothing in this history to suggest that this is HSV.  HSV does not cause prostatitis, does not last as long as your symptoms have and does not cause urethral pain without there also being external lesion.  This is not HSV.

I hope these comments are helpful.  Typically problems such as prostatitis require a more prolonged course of therapy with a medication such as trimethoprim-sulfamethoxazole.   EWH

I had to wait a few days on the test.  It will still be another few months before i can see a urologist unfortunately.  keep fighting the good fight

Friend, I had the exact same issues and recently saw a urologist. He prescribed bactrim. I thought it was chlamydia but my tests were negative and like you there was no discharge. My issues are also lingering...

Chlamydia is rapidly cleared up with Azythromicin and usually doesn't result in lingering issues however standard NGU can reoccur however like the doctor said it sounds like prostatitis.

Did you have to wait for your test results or did they tell you immediately?

Doc,

A urinalysis test of some form was conducted to diagnose me with chlamydia.  I trust it as it was done in the States, at a reputable hospital.  Perhaps it was coincidental that my symptoms appeared.  Again, I had no discharge to speak of.  My symptoms now are urethral pain, occasional pain in what i imagine is the general area of the prostate, and a sense of incomplete emptying of the urethra.  I do not remember having any external lesions, so I put herpetic urethritis low on my list, but there is a great deal of contradictory information on the subject which brought in an element of doubt.
