Contagion through hands on condom

Hi Doctor , sorry for my English, but i am Italian.

Please Doctor help me beacuse I am very worried. I am male, 28 years old and in the last month i received 4 times UNPROTECTED oral sex by african and Chinese prostitute, 1 time UNPROTECTED oral sex from a brazilian transexual, 2 times PROTECTED vaginal sex. These are the only episodes that have occurred in the last 4 months, never unprotected vaginal or anal sex!

Now, from about 2 days, i have a redness around the urethra, burning during urination and a slight (usually clear) discharge (drip) from the urethra. i think is NGU...

Now I am very worried about my girlfriend; i had sex with her 3 times in the last 2 weeks, always only vaginal PROTECTED sex, NEVER (NO) UNPROTECTED ORAL. The problem is that yesterday I masturbated myself before putting the condom on me and have sex with my girlfriend, but my hand was not clean, maybe there was a little discharge on my hand (maybe, i can't be sure) and maybe i pass this discharge on the condom when putting it...  I'm sure I rubbed my hands on the outer surface of the condom before having sex..
I infected my girlfriend this way?
how much are the odds of infecting my girlfriend?

I'm terrified for my girlfriend ... i can't make test for std and Today I took 1 g of azithromycin and 500mg ciprofloxacin. With these antibiotics, I cured my infection?
I am very concerned,

Sigh.  You keep giving different signals.  The small amount of clear discharge usually means nothing, but you you talk about painful urination -- and the eye problem.  These might suggest adenovirus, which can cause simultaneous urethritis and conjunctivitis.

It is time for you to totally stop trying to figure this out by online searching or question, either on this forum or anywhere else.  See a doctor.  I will have no further comments or advice of any kind until you report (in detail) what has been found after you have been professionally evaluated.

Last question: I have a very strong pain when I urinate and now i have a conjunctivitis in the left eye, all these can be linked?

Scant clear discharge generally is not abnormal; you may just be looking too closely.  Your negative tests probably mean you were never infected, but it is possible you had been infected and cured.  As discussed above, there is no way to ever know for certain.  Don't waste your money on any future tests.

Herpes is not likely; if that had been the problem, it wouldn't have improved this much so quickly.

If your discharge continues more than a few more days (even scant and clear), you and your partner should both see a doctor or clinic.  Do not treat yourself with anything before being examined.  After it clears up, it probably will be safe to have sex with your partner.

ok Doc, I've the results of my tests:

- semen for strep-staf-enterobacteria-fungi-gonorrhea-Mycoplasma ---  all negative
-urethral swab for strep-staf-enterobacteria-fungi-gonorrhea-Mycoplasma-chlamydia --- all negative

The problem is that I still have clear discharge (resemble a drop of urine) from the urethra and slight burning during urination...

1)I took antibiotics (azi and cipro) 36 hours before doing test, do you think I could consider myself free from diseases?

2)I've to be afraid of a false negative test? Is better to repeat test in 10/15 days?

3)If is herpes, there is no cure, how long should I abstain from relations with my girlfriend? I have to wait until the symptoms go away?

Azithromycin sometimes causes vomiting.  But if more than 30 minutes after swallowing, it's not a problem; the drug is still fully effective.

That's my last comment until you report your and your girlfriend's test results and the doctor's or clinic's evaluation.

My girlfriend want to take antibiotics...

But please doc: what about vomit after 70 min of taking antibiotics?

We are in Italy now, se get test this afternoon!

Excellent -- congratulations for getting up the nerve to discuss it with you girlfriend.  But why do you keep insisting on treatment before testing???  That was also a mistake and might make her test for gonorrhea/chlamydia negative even if she is infected -- but it may still be useful if done within a few hours.  I hope you're seeing a doctor or clinic, and not testing yourselves in a lab.  It's important you both be examined by a professional.

No more comments until you let me know the results of both testing and the doctor's or clinic's advice.

Ok Doc, I have talked with my girlfriend and this afternoon we're going to get test.

Only a reassurance: this morning my girlfriend took 1 g of azithromycin and 500mg ciprofloxacin and she vomited after 60-75 minutes,
there may be problems with effectiveness?

On this forum we don't examine users' clinical photos; I deleted your comment with all those links.

If your symptoms improve promptly within 2-3 days, it will probably indicate you had a bacterial infection.  But sometimes it takes longer; and even if things improve, it doesn't necessarily mean the antibiotics were the reason.  If you have herpes or other viral infection, antibiotics will make no difference but the symptoms will clear up on their own within 1-2 weeks.  And if you indeed have inflamed lymph nodes in the groin, that makes me even more suspicious you may have a new herpes infection.  None of the other conditions we have discussed would cause lymph node inflammation.

You are caught in an age-old dilemma:  what does the straying partner say to a spouse or lover when an STD problem arises because of an outside relationship?  I'm afraid I don't have any easy answers for you.  The chance you infected her, given the condom-protected sex, was low.  But I cannot say it was zero.  And I would never recommend coercing a partner into taking an antibiotic without being truthful about the reason for doing so.  And if your problem turns out to be herpes, azithromycin or other antibiotics won't help anyway.

My advice is that you 'fess up, and the two of you go together to a knowledgeble clinic or doctor, then follow the advice you receive there.  You probably blew the chance for proper diagnosis and medical care by treating yourself without medical advice, but your partner doesn't have to do that.

If and when you and your girlfriend have both been professionally examined, and you know the outcomes for both you and her, I may have more comments.  Until then I won't have any further advice.

sex with my girlfriend(NEVER ORAL SEX; JUST VAGINAL PROTECTED SEX):
1- two times during first and second week of October, I do not remember the exact dates
2- 14 october
3- yesterday 21 october

symptoms beginning on 14 october: after sex with my girlfriend, just very little nuisance on the glans. After 2 days started very very light burning urination, that yesterday and today has become sharp burning (just around the urethral opening). yesterday morning i noticed discharge (drip) from the urethral, a very small drop of clear liquid. the redness around the urethral opening is not "strong", is just a little redness in comparison of the rest of the glans. burning sensation becomes stronger during urination; I feel a sting in urethral opening.

Doc, I can not explain all this to my girlfriend;
What are the chances of infecting my girlfriend in the 4-5 always protected sex we had? (considering I rubbed my hands on the outer surface of the condom)
What do you think if my girlfriend takes 1g of azithromycin?
If tomorrow I have improvement in symptoms it means that the antibiotic is correct?

Welcome to the forum.  Thanks for your question.

I agree your symptoms are typical for urethritis, i.e. infection of the urine passage.  I'm equally concerned you have gonorrhea as NGU; or perhaps a viral urethritis from adenovirus or herpes.  All these are possible from unprotected oral sex.  There is a much lower chance of chlamydia from oral sex, but it's still a small possibility.  (It is the redness around the urthral opening that makes me concerned about gonorrhea and viruses.  Standard NGU and chlamydia usually don't have this.)

What is the timing of your symptoms compared with your various sexual exposures?  Tell me as precisely as you can when you last had unprotected oral sex outside your relationship, and also the timing of your most recent oral and vaginal sex with your girlfriend.

You should not have treated yourself with antibiotics before being examined.  That was a very big mistake!  As a result, it will be difficult or impossible to ever know the correct diagnosis.  If you can manage to get to a doctor or clinic knowledgeable about STDs (preferably a public health STD clinic) immediately -- today or, if necessary, tomorrow -- gonorrhea and chlamydia testing MAY still be helpful.

If you have gonorrhea, chlamydia, or garden-vareity NGU, the combination of azithromycin and ciprofloxacin probably will cure it.  But they won't have any effect if you have a viral infection like herpes or adenovirus.

Although you have used condoms consistently with your regular partner, because you had sex with her since you acquired your infection, I would still recommend that she be examined and treated.  The chance is low, but unrecognized condom failures do happen and you probably don't want to take a risk of endangering your girlfriend's health.  However, the chance you infected her by hand-genital contact, or by how the condom was applied, is very low.

I may have more advice after you provide more details on timing of your sexual exposures and symptoms.  In the meantime, I hope this starts to help -- even though some of the information probably isn't what you hoped to hear from me.
