Right Medication(s)?

Hello,

A little over 2 months ago I had a sexual encounter (penis - vagina) with another soldier - unprotected.  Later on, my concern was whether or not she may STD; especially after I discovered she had encounters with a few other soldiers here.  I never really seemed to have any symptoms of anything until the concern came up.  Even then, I never experienced any significant symptoms.  About 4 1/2 - 5 weeks later I started to experience what felt like a sharp, shooting itch in the penis area that would only last a second.  6 weeks after the encounter I woke up in a panic as it felt like I had an itching sensation in between my scrotum and anus (but this feeling was internally).  I rubbed the area down there and then felt the tip of my penis and panicked again because I felt a sticky substance that I believed could be a discharge.  I spoke to a doctor. I was unable to produce a discharge for him until I rubbed where my prostate is and then a slight, somewhat clear, discharge became visible.  He put me on the following anti-biotics:
Erythromycin 4x's for 7 days)/ Cipro 2x's for 7 days.  I was already on Doxycycline for prevention of malaria. I haven't really had any symptoms since I completed medication, which was about 2 1/2 weeks ago.  No painful urination, no itching from the urethra, etc.  I would occasionally feel that sharp, shooting itch in the penis area for a while after that.  I also noticed when I had bowel movements that I would have a discharge that resembled semen and sometimes I would seem to have a slight delay when I'd have to go to the bathroom (like the urine would be held for a split second before shooting out). No burning or itch though.  I went to the clinic again and was told by a different doctor that he believed that I may have prostatitis and prescribed Septra 2x's for 7 days.
Is there any chance I could be suffering from mild or asymptomatic gonorreah or chlamydia infection or would the antibiotics I was taken have cleared that up?

You have little to worry about for several reasons.  First, other than the discharge you describe, the symptoms you describe are not suggestive of any STD.  As for the discharge, rubbing in the prostate area is expected to produce a discharge of prostatic secretions - we use this as one of the tests for prostatitis.

Second, if you were on doxycycline for malaria prophylaxis, it is unlikely that you got infected, even if she was.  The doxy would prevent it.

Third, between the various antibiotics you've taken, if you had chlamydia, nongonococal urethritis or gonorrhea, they would have been cured.

Fourth, if you've been tested through the military (you didn't mention whether you were or not), and your tests are negative, this is good evidence that none of those infections are present.  The tests used for STD diagnosis by the military are very good.

Bottom line, the symptoms you describe do not suggest and STD and your history suggests that your risk for STD is very low.  Hope this helps.  EWH

I think that the default assumption when we get urinary/penile symptoms is that we must have an STD.  In your case, I can promise you that this is not the case.  The therapies you have had would have cured any STD several times over.  As to what else might be going on? Hard to say.  Kidney stones?  Drug reaction?  Other??  The best way to address your continuuing sypmtoms is to go on to your doctor as you will tomorrow.  Please let me know what he/she says.  EWH

It has been some time since my original message, Dr. Hook.  I just hope you can offer some advice to me now before I attempt to go see a physician tomorrow.  The website has been a LOT of help and I've read over many of the other post.

My biggest problem(s) as of late have been that after I urinate I feel urine trapped in my urethra that I have to go back to the bathroom to try and force out 1 - 2 minutes after voiding and it may have to be repeated a couple of times.  If I don't do this the feeling remains for a considerable amount of time.  I had also noticed an annoying itching feeling when my penis rubbed against my boxers.

While at Bagram Airfield, (in Afghanistan) where I could see a doctor(s) with access to lab equipment I explained my previous use of erythromycin, doxycycline and ciproflaxin figuring that would eliminate at least a couple possibilities.  The physician I saw wanted to do testing for STDs but was informed it would be 6 weeks before results would come back so she just prescribed me a shot of ceftriaxone and gave me some flagyl (single dose) to eliminate trich.

I don't know what these doctors are going to tell me tomorrow.  I am stateside but have about 10 more days before I go back to Afghanistan.  What COULD be my problem?  It seems like an STD (bacterial at least) is out the question with all the meds; however, the discomfort I feel in my genital reason is REAL.

Psycologically, you do not know how much you helped me.  I was a mess, but don't really think I had symptoms of an STD.  I finally found the courage to ask the young lady that I had the experience with if SHE felt anything and she hadn't.  I just seemed to feel EVERY symptom possible....probably due to anxiety, even though the specific symptoms for gonorreah and chlamydia weren't there.  I just read so much on the internet about the possibility that there are asymptomatic conditions that I wanted to be sure.
I heard that Cipro was a one dose, 500 mg. cure for gonorreah (I took 2 doses a day for a week); however I had heard that some strains of gonorreah could be resistant and that erythromycin and doxycycline were strictly for chlamydia.
I have been reading other post and I think that what I am about to ask you could already be answered, but I will ask it anyway.  The only reason I did not ask is because of restraints.
I had a rash on the scrotum and the inner thigh (and a little anal itch) that persisted and I attributed to jock itch.  I used a spray (probably too much) to treat it.  The rash on the thigh AND the scrotum went away but I also used it on my penis, due to the sharp, split second itch I kept feeling in the penis area.  My penis seemed to get irritated so I stopped using it.  I especially seemed to have an irritation around the tip of the head of my penis.
For some stupid reason I sprayed my entire genital area with OFF because of a slight tingling and itch that I THOUGHT I felt on my penus and I KNOW I felt on my anus.  It burned my anus but the only effect it seemed to have on my penis was a small area where I noticed scabs on the shaft of my penis.  My whole penis (and scrotum) seemed irritated.  I stopped using the spray and noticed that the discomfort lasted for two days.  I  started using protroleum jelly to soothe my genitals and it felt completely normal after about three days.  It might have been less...but I was beginning to get worried about genital herpies.
Do you think I am good for herpes.  I hope you can reply.  For $15 I hope I am not wasting your time with this question.  The scab(s) came off and left no visble mark and I never noticed anything that resemled a lesion.  It just hurt down my shaft.  What are your thoughts?
