STD/HIV risk from protected oral sex and fingering

Hello Dr,
I had a protected oral sex on May 07, 2010. I fingered her while she was performing oral on me. After fingering her, I touched her breasts with the same finger and I sucked her breasts. I am not sure whether her vaginal fluid went inside my mouth while sucking her breasts. She also licked my thigh area before performing oral on me. After 4 weeks later, I saw a small lesion on my thigh area, but it went away next morning while taking bath. I also felt uncomfortable with my mouth when I eat spicy foods, I felt itchiness in my mouth at that time. I thought I contracted oral and genital herpes from the above exposure and went on to take the WHOLE STD package after 4 WEEKS and all came back NEGATIVE. I was also feeling headache after 6th week which lasts for 1 week.

Then after 8th week, I feel tingling sensation in my penis. Its kind of cell phone vibration in my penis. I was terrified. I ruled out Gono, chlamydia, since the test was taken after 4 weeks, i also ruled out syphilis, since in OR, the rate is very less.

So I went for HIV and Herpes 1/2 HerpeSelect after exactly 8 WEEKS and both came back NEGATIVE. I know HIV test is conclusive after 8 weeks with the kind of exposure i have, So now I only terrified with herpes. Do i need to take another test after 4 months? What is the reason behind the feeling of vibration in my penis which lasts for 5 seconds and come back after 10 min and continue throught the day? It is annoying..

Your risk for any STD remains negligible however, to finish up your evaluation, please see my comments above about NGU. You need to be tested.  If there are no WBCs you do not have NGU and the secretions you see are normal, if there are, you do have NGU and should be treated.  The tests for NGU are best performed if you have not urinated for at least an hour before the specimen is collected.I predict the tests for NGU will be negative unless the trauma of your repeated self examination has caused inflammation

Finally, please stop repeatedly examining yourself.  If does no good and can make things worse.  EWH

Welcome to our forum.  I note from your other posts that the folks on both the HIV and STD Prevention community sites have told you that this is a no risk exposure and that there was no reason for concern. You have now had those well informed opinions validated by a series of negative tests.  I agree with the advice that you have already received that you did not get an STD of any sort through this exposure and see no reason for concern and no reason for further testing.  

As for your symptoms, not uncommonly after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turn leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  I suspect that this is a contributor to your situation.   My advice is for you to believe the advice you have had and your lab tests.  You do not have a reason for concern and do not need further testing.  I hope my comments will allow you to stop dwelling on this and move forward.  EWH

One last time (and this is the last time, further questions will be deleted without comment), there was no risk for STD from the exposure you described, your symptoms are not suggestive of STD and your tests have repeatedly shown that you do not have STD.  You need to stop worrying about STD and deal with your guilt and anxiety.

As far as you "tingling" and probably other symptoms, pllease re-read my orignal answer. it still holds.  EWH

Sorry for adding one more qn.

3. I am also having some pain in my left eye and it started 2 days back. Is this symptom of any STD that the test didn't cover?

I got my 11th week results back and thank god, all came back NEGATIVE. I can now relax a little bit.

You are right, these are the last questions as these are also the last answers,.  Your anxiety is getting out of control.

1.  No, this sounds nothing like HSV.
2.  As far as I am concnerned there is no reason to test for HSV. Thus whenever you take a HSV test, you should take it as conclusive.

Hello Dr. Thanks for your reply. I assure you that this IS my last question.
I am not worried about NGU, because it is treatable, I am just worried about Herpes. Here i s my final 3 questions:

1) Is having dark thin black circle around the opening of penis symptoms of Herpes? I don't know if those circle was there previously or not. I read somewhere that if you have a dark black circle on the tip of penis, it might be a symptoms of STD. Is that right?

2) If I take another herpes test after 12 weeks and if It come back as NEGATIVE, can i move on with my life?

3) For the last 2 days, I dont feel the tingling sensation on my penis, it was there for 2 weeks. Do you think is it asymptotic herpes outbreak?

I would like to add one more thing. This happens only at the morning when I wake up and before urinate. It is not constant and I have to squeeze the penis to see the discharge.

Hello Dr Hook! Here is my last follow up question.

For the last 2 days, I dont feel the tingling sensation in my penis, but in the morning when I squeeze my penis, I see only one drop of discharge in the color of water, but it not looks like water or seman, it is very thick. What could be this? I am really terrified. I am getting married in 3 months. I just want to get over this. Thanks for your help.

Your syphilis test is conclusive.

The test for NGU is to look for the presence of white blood cells in a swab test taken form the penis or in the intial portion of a urine specimen (i.e.  collected just as you start urinating).  If there is not an elevated amount of WBCs you do not have NGU. EWH

FYI, I had tested negative for Gono, Chlamydia after 4 weeks, it didn't include NGU. Is there any test for NGU that i can take to ruled out NGU? Is syphilis test conclusive after 8 weeks? Thanks Dr. This will be my final question.

Thank you very much Dr. I can move on with my life if i had no strange feelings on my penis. It started after 8 weeks. Did I take a herpes test too early (8 weeks) or is it something like NGU or syphilis? I couldn't able to concentrate on my work. I always feel something is going on my penis. I never had such a sensation ever in my life. I am terrified.

1. What are the chances that it could be NGU? Is there any test to find out if it is NGU? If so, what is the name of the test? What is the window period for that?

2. Can I take syphils test now (after 8 weeks) to ruled out syphilis?
