STI/UTI from massage

I went two an erotic massage parlour two weeks ago, and had a hand job that lasted about one minute before I stopped her. Within a day I was feeling frequency and mild dysuria, so I went to a clinic and was given Azithromycin and Cefixime. They also did a urethral swab and urine c+s, however (unfortunately for my anxiety) those swabs and results have been lost by the clinic.

Over the past two weeks I've become increasing worried about STI exposure. I've had persistent frequency and dysuria. I know you've spoken many times about genitally focused anxiety on this forum, and I suspect this could be playing a role.

But, here's the catch: I went for a repeat urine c+s last week since the previous results were lost, and it was negative for G and C but came back positive for enterococcus faecalis. I told the physician about my exposure, and he recommended treatment with macrobid twice a day x 5 days. I was convinced my symptoms were related to anxiety, so needless to say I was surprised by this result. I can't recall the colony count exactly, but I think it was 20 x 10'6.

For you purpose, I'm a 23 year old white male with no past UTIs, no past STIs, and past medical history is only relevant for anxiety, IBS and perianal fistula. I'm currently in a committed two year relationship with another woman. I had a negative STI workup prior to this relationship.

Here are my questions:
1) Is there any STI risk from my encounter. G+C are negative, my readings on your forum indicate herpes/HPV risk is non-existent/rare, HIV impossible...but I am particularly curious about syphilis.

2) I'm trying to figure out why I got this UTI and if it was sexually transmitted? No anal sex hx. I'm curious if it could have been transmitted by sex (either by the massage or my gf), or that my past antibiotic treatment made me predisposed, or if its contaminant?

3) Would you recommend further treatment, and is my girlfriend at any risk?

It would be unusual to have a symptomatic UTI with no nitirites, WBCs or leukocyte esterase.  EWH

I actually never saw the urinalysis, only the C+S results, but ill try to track it down and post it when I know. The initial urine dip thought showed no leuks, nitrites or anything else for that matter.

Welcome to our Forum. I'll be pleased to comment.  I appreciate that you have read other interactions with clients about genitally focused anxiety and your self-analysis of your own situation.  I think that your intellect and your emotions are taking you in different directions.  There are simply no STIs that are transmitted by masturbation and thus the event you describe was no risk for STI.  I suspect that your concern, perhaps guilt, and attention to the possibility of symptoms have contributed to the symptoms that you have detected although I have some questions about your positive urine culture (see below).

As for your positive urine test, it brings to mind a comment I make often as I train medical students and physicians in training.  When yon perform a test, you should know what you are going to do with the results (positive or negative) before you order a test.  It is no surprise to me that your doctor found enterococcus in your urine- it is normally there although the quantity that you mention is a higher than normal (are you sure there were 20,000,000 organisms per milliliter of urine- this is extraordinarily high)  and ay warrant treatment, particularly if there were other abnormalities on your urinalysis such as a positive leukocyte esterase test or increased numbers of white blood cells.  For most situations the preferred treatment for enterococcus is amoxicillin.

As for your specific questions:

No, there is no meaningful risk for any STI, including syphilis from this encounter (and your symptoms are nothing like those associated with syphilis)

UTI's are unusual in men of your age.  It may have been a contaminant although the symptoms (urinary frequency) you report are consistent.  IF the urinalysis was abnormal, then this may warrant further investigation and I would suggest you discuss this with your doctor. It may warrant further evaluation of your prostate.

3) Would you recommend further treatment, and is my girlfriend at any risk?
No, the fact the you have enterococcus does not impact your GF in any way and does not warrant treatment.
