Testicle Pain, No STD. Please help.

A little over a year ago I started having symptoms of an STD. (itchy urethra, clear discharge, and burning during urination). I was tested (swab) and was given azithromycin and another antibiotic. I believe it was 5 pills total. Within a day or so my symptoms were gone, so I didn't call back for my results. A month or so later, only the discharge came back. I have since then only had one partner (girlfriend). About two months ago I started experiencing testicular pain. I went to a clinic thinking that I had epididymitis. The nurse checked me physically and didn’t give me a diagnosis. I received a shot of rocephin and sulfamethoxazole and the pain went away, as well as the discharge. I finally felt normal. Three weeks later the testicular pain (no discharge still) came back and I rushed back to the clinic, I was given a swab test,shot of rocephin, and doxycycline. The doxycycline made me very nauseous and caused me severe vomiting. I discontinued the use of it and was given levofloxacin. Meanwhile, the std swab tests came back negative. I didnt take the levofloxacin, because I wasn’t sure if I even needed it since I havent been properly diagnosed. I made an appointment with an actual Doctor. He physically checked my testicles,checked for hernia, seemed fine. I also had blood work and urinalysis done, results were good. He told me to not take any more antibiotics and prescribed me clonazepam for anxiety. He told me to come see him first if the pain got bad again. An ultrasound showed I have small bilateral hydroceles. My doctor told me to take aleve for pain along with my clonazepam. I still have dull achy pain in my testicles, some days it’s worse than others and is sometimes made worse by straining, or bending over. Laying down make the pain go away. Yesterday, the discharge came back. I have not been sexually active since before my swab tests. I'm tired of being consumed with anxiety and worrying about my health. I need advice. I want to feel normal again.

1. The clinic I went to  gave me a shot of Rocephin and the Sulfamethoxazole for 7 days BEFORE I was given a swab test on the second visit. Wouldn't these two medications cure the 3 STDs I was tested for, resulting in the negative test results?
Yes most probably.  

2. My testicles do not appear to be swollen due to the hydroceles, they look normal to me. I was under the impression that hydroceles only become bothersome when they become quite large.
They are an anatomic abnormality.  Such abnormalities can cause a variety of problems.  

3. Would the blood work or urinalysis show signs of prostatitis?
No.

4. I agree, that this has felt like a guessing game and a process of elimination. And, unfortunately has cost a lot of money considering that I'm uninsured.

5. Should I go back to my doctor and request an examination of my prostate at this point, or should I see a Urologist.
You could start with the doctor who has already seen you.  EWH

Thanks this is helpful and in the aggregate reduces the likelihood that these symptoms, including your discharge and testicular pain were due to any STD.  Had they been due to one of the STDs which typically causes the syndrome most associated with testicular pain, epididymitis your STD tests would have been positive.  

There are numerous non-STD causes of testicular pain and of them prostatitis is the problem most associated with the combination of testicular discomfort and discharge.   With all due respect, so far your "care" appears to have mostly been associated with guesswork and empirical antibiotics provided based on your clinicians' best guesses and partial exams.  Possibilities for what might be going on here that occur to me is that this might be related to the bilateral hydroceles you mention or perhaps prostatitis.  It is now time to bite the the bullet and work with a single physician who you feel comfortable with to address these persistent problems (it is not normal for such problems to persist in this way for over a year).  Evaluation at this point should go back to the starting point and be comprehensive, including testing and a rectal exam to evaluate your prostate.  The doctor’s advice to stop antibiotic is also good advice.   Finally, although the possibility of STDs is quite small, you could also help yourself if you can have both of your partners, tested for common STDs (as opposed to “throwing” more antibiotics at them), not because they are likely but to be sure that something unlikely is not going on (and if you say it this way, you do not have to deal with explanations, etc).    EWH

additionally..

i went to a clinic late February 2013 with the testicle pain. I was given a shot of rocephin and 7 days worth of sulfamethaxazole. The pain went away for 3 weeks.

3/16/13 I was back at the clinic with testicle pain. I was swab tested for gonorrhea, trich, and chlamydia. While I was there, i was given another shot of rocephin and 20 days worth of doxycycline. I only took two days worth of doxycycline before it made me extremely nauseated and severe vomiting. My antibiotic was changed to Levofloxacin, but I never took them. My test results came back a few days later and they were all negative. Maybe because of the antibiotics given to me in february?

3/21/13 I went to an actual Doctor. Checked for hernia, examined my testicles, did bloodwork and urinalysis. He told me to discontinue any antibiotics.

3/22/13 I went and had an ultrasound performed. Discovered I had small bilateral hydroceles.

Thanks for the prompt reply. When I first had the symptoms a year ago (itchy urethra, burning while urination, and clear discharge) I got tested but never called back for results because the symptoms went away after the azithromycin and the other antibiotic that I cant remember. So I dont know for sure that I had a STD. My girlfriend was given doxycycline by her lady doctor, after hearing of my symptoms. I have had two sex partners in the last two years. (my girlfriend, and one other) I started experiencing the symptoms above after sleeping with the "other" after me and my girlfriend broke up for a short time.

Welcome to our Forum. I'll try to help.  Please provide me with a bit more information.  You mention many courses of antibiotics but no test results. Have you EVER been proven to have gonorrhea, chlamydia, NGU or another STD?  In addition during this period, have you had a single sex partner or multiple partners and have there been times when your partners have been treated at the same time you have?  Both the symptoms and your responses to therapy sound quite atypical for most STDs and certainly for epididymitis.

I will have more comments following hearing a bit more form you. EWH
