Unprotected Oral, Vaginal Fluid

Dear Doctor,

I had unprotected oral sex with prostitute.  This is how it went.  I fingered her vagina and then she fingered her vagina.  Then she masturbated me and then she gave me unprotected oral sex.  After oral, I noticed a small group of eraser sized black dots on her inner thigh.  Then I was done.  She probably touched the area and touched me during our oral as she seemed intent on rubbing herself and working and sucking my penis.  I have no idea what this means.  It's been 5 days or so and nothing has happened, but I'm freaked because I had sex with my significant other 3 days later.  I had no idea you could get stds from oral sex until I got paranoid and started looking up things on the net.  Had I know I would not have continued to have sex with my significant other.  So I guess the question of risk resides around the vaginal fluid, those "black hard dots" (which could have been some kind of healed scar thing) and saliva.  Not sure how to proceed.  I emailed the "escort" and she said that she tested regular and was clean (but who knows).  Please respond.

Urinary frequency in women often means a non-STD urinary tract infection, especially if accompanied by one or more of urgency ("gotta go right NOW"), small urine volumes, and burning pain on urination.  All by itself, frequency probably is usually not due to any infection at all.  STDs can cause some of the same symptoms, but again, not frequency alone.

Is it conceivable that you caught an oral infection, and transmitted it to her by oral sex?  I suppose so -- but that is the only possible mechansim.  And the chance of that, in view of the nature of the exposure and the other things we discussed above, are astronomically small.  In any case, if your wife's urinary symptoms continue, of course your wife should see her primary care doctor or clinic for professional assessment.   If she should happen to have an STD -- which is extremely unlikely -- it will have nothing to do with the oral sex event.  Same for you, in the event something turns up if you get tested.  Therefore, for now you need not mention it.

My wife complained of frequent urination today.  If you are still somehow watching this thread, I guess I'm curious as to how long it would continue to occur if in fact i have given her an STD.  She didn't complain of anything but the frequency.  I guess I'm a little anxious.  I am contacting a private clinic tomorrow for testing even though i have no symptoms.  Thanks if you are still out there.

Thanks Doctor, I appreciate your input.  This stuff is crazy.  I understand more about anxiety now too, but I'll trust your advice as I was completely honest with you about what happened.

Welcome to the STD forum.

Your initial belief about oral sex and STDs was incorrect, as you learned.  However, that belief may still have been closer to the truth than what you learned on line.  Many online sources would make you think oral sex is just as risky as vaginal or anal sex, which is wrong.  There is little or no risk for some STDs (HIV, chlamydia, HPV, HSV-2) and quite low risks for others (gonorrhea, nongonococcal urethritis, herpes due to HSV-1, syphilis).  Further, any of these STDs acquired by oral sex usually cause noticieable symptoms -- and therefore testing for STDs usually isn't considered necessary after an oral sex exposure.  And fingering or other indirect contact with genital secretions also carries little if any STD risk.

As for the "black dots" your partner's thigh, no STD causes such a thing.  Most likely these were birthmarks, moles, or something like that.

Further, most escorts -- i.e., expensive sex workers who work by appointment -- are aware of STD risks, know how to protect themselves, and tend to be at relatively low risk for STD; and it sounds like your was tested recently and not infected with anything detected by routine testing.  (Of course people can lie about such things, but most are truthful when asked directly.)

For all those reasons, it is very unlikely you caught any STD or put your regular partner at risk.  If I had an exposure like this, I would continue unprotected sex with my wife without fear of infecting her.  Given your nervousness about it, you still might want to see a provider knowledgeable about STDs and get tested, but it's optional.  If so, the only tests that make sense are a urine test for gonorrhea (valid any time more than 2-3 days after exposure) and blood tests for HIV and syphilis after 6-8 weeks.  If you decide on testing, you really should be pretty relaxed as you await the results.
