I phoned teh doc and he informed me that no bacteria was present and i was clear. I finished the antibiotics two days ago ad now i have small spots on my penis head and just the foreskin area. They are very small but i dont know what theyare. Do you have any idea? what STD's could cause this (16 days after event) and should i go to doctor again?

You have apparently had a single exposure to someone who may be at relatively high risk for STD and possibly HIV. You are unaware of all the details so we have to start with some generalizations about risk and then go from there. Most commercial sex workers (CSWs), despite being at higher risk, do not have STDs or HIV. In addition, a single exposure to an infected sex partner usually does not result in infection (the transmission rates for different STDs differ between them but for female to male transmission, none of the rates exceed about 20% per episode of intercourse and for some, like HIV, the risk is far smaller, like 0.01% per sex act). Thus you can take some comfort there. Now lets move forward to what we know about. In doing so, you mention several medications I am unfamiliar with so I need to ask you to clarify if possible.
1. Zithromax is azithromycin. It is recommended, curative therapy for chlamydia and NGU. In addition, the dose used for chlamydia cures some but not all gonorrhea as well. Finally, taking it will likely prevent development of syphilis if you were exposed.
2. My guess is that this is another name for ciprofloxacin, a medication which will cure most but not all gonorrhea (some strains are resistant). If you got gonorrhea and the ciprofloxacin did not cure it, you will most likely develop a discharge from your penis in short order (a few days).
3. I'm not sure what this is but yes, you should always tell doctors what other medications you've taken.

5. No. Once the infections are cured there is no further evidence of infection.
6 No, you should be OK, given the caveats above.
7. This is beyond the scope of this forum however, if you are a problem drinker and it sounds like you may be, you need the support of at least your family and then to seek discrete help. IN the US that would be alcoholics anonymous.
