Unprotected encounter - symptoms question

- June 23rd I had unprotected vaginal sex with a young lady I met at the bar, no idea of her sexual history  

- Day 2: some discomfort when urinating & some redness in my urethra. Went to STI clinic and tested for chlym. and gonor. which came back negative, but due to lots of water drank, urine was very clear and might have been a poor sample. I was prescribed a single dose of cefixime and a week's worth of doxycycline.

- D3: very painful urination in the AM. Drank lots of water and urinated often, when my urine was clear it didn't hurt

- D4: Peeing without discomfort.  In the PM I checked my urethra and when I pulled it, the skin was dry and tore a little bit at the top causing some bleeding. The sore healed within a few days. I drank a ton of water over the next week, and was working outside in extreme heat and urinated very frequently without pain.

- D11: Small red bumps showed up on my penis and my foreskin was red. Felt warm but not painful.   Went into a walk-in medical clinic and was prescribed lotriderm topical cream. Doc thought fungal infection due to lots of urinating outside in extreme heat, penis didn't have a chance to dry.

- D12: Tongue was a little sensitive to spicy food and found that my fungiform papillae were red and a tiny bit swollen.   Very similar to the picture on this page: http://tinyurl.com/25mbpp8   Seems to me that the back of my tongue might have a bit thicker white coat than is usual, but this could be my imagination.   I have been very stressed about this situation and have had a very, very dry mouth.

- D18: Rash on penis has reduced, but small red bumps still visible, especially when penis is erect. Tongue still has enflamed taste buds, no change, slightly sensitive to spicy food, no bleeding or open sores though, very dry mouth. Have had yellow diarrhea, bright not pale for about a week.

Could a male yeast infection combined with stress, antibiotics and dry mouth cause all my symptoms or likely something else?

This indeed sounds like a photosensitivity reaction, a common doxycycline side effect, especially in the summer and outdoors a lot.  It can take a couple weeks to clear up.  Most cases are harmless, but do your best to avoid sun exposure and/or use a strong sunblock (30+ spf).  See a dermatologist if it hasn't started to clear up in the next few days, or any time if you think it's actually getting worse.

There are several potential explanations for the voiding discomfort after a casual, apparently high risk sexual exposure.  Not all of them are STDs.  A physical or chemical irritation of some sort is more likely for symptoms that start within only 1-2 days after exposure.  In any case, the negative tests for chlamydia and gonorrhea rule out those possibilities, and the treatment you had would have cleared up (or prevented) chlamydia, gonorrhea, and most cases of nonchlamydial nongonococcal urethritis (NGU).  Since those symptoms have now resolved, they should no longer be of concern.

None of your other symptoms is likely due to any STD, which would not cause the tongue changes you describe.  The tongue whitening was probably the result of the antibiotics.  No STD causes taste buds to become more prominent.  The sort of superficial red bumps of the penis suggests a fungal infection; and since that's what your doctor thought on direct exam, I see no need to look for other explanations. This also could have been triggered by the antibiotics, but almost certainly was not sexually acquired.  

In summary, in the slight chance an STD explained the urinary discomfort, that problem is now is likely.  If symptoms of any kind persist or you remain concerned, return to the STD clinic or other health professional.

Final advice, as if you didn't know:  please get into the condom habit for similar events in the future.

Forgot to add:

Day 3: I noticed a lot of small red spots (severe freckling or petechial rash??) on the back of my arms.   It looks like spotted bruising.   I was in the sun a lot that day, but kept my shirt on the whole time.   The rash starts from the bottom of my sleeve to my elbow on both arms.   This was the least tanned skin that I left exposed that day.   I stopped by the STI clinic and showed it to a nurse but didn't seem concerned about it and said it was probably just a photosensitivity reaction to the medication.

2 and a half weeks later, and 12 days after finishing the doxycycline cycle, it is still there, just as noticeable as before.   Is this freckling/petechial rash from the sun a common reaction to doxycycline?   How long should I expect it to last?   Is this something I should bring to the attention of a doctor?   Should I be protecting it from the sun for now?
