stressed over recent exposure

Hello doctor, my question involves a recent experience with an escort i had. The activities are as follows: i performed light uncovered oral sex on her vagina, recieved an uncovered handjob in which some vaginal fluid may have been on her hand, and a golden shower, small amounts landed in my eye and mouth however i didnt swallow. Can you please advise me of HIV or other std risk and how risky this is? Thank you for all your help.

Welcome back to the forum and thanks for your question.

I fear you did not understand the nuances of Dr. Hook's reply to a similar exposure (condom-protected sex with an escort) a few months ago.  The bacteria and viruses that are transmitted sexually evolved that way because they rarely are able to cause infection by fleeting, superficial contacts.  Transmission requires direct exposure of substantial amounts of the organisms to susceptible tissues.  With rare exception, if a bare penis (no condom) does not penetrate another person's vagina, rectum or (fairly rarely) mouth, there is little or no risk.  This is especially true for HIV.

So as for a few months ago, you should not worry about the events described.  Urine is pretty good at killing bacteria and viruses, so urine exposure rarely carries any risk; your "golden shower" should not be a concern, regardless of possible eye or mouth exposure.  And cunnilingus (oral-vaginal contact) carries little STD risk for either partner.

If you remain concerned, you could visit a doctor or clinic for examination and testing.  But I really don't think it is necessary.  If I were in your circumstances, I would not be tested for anything and I would continue unprotected sex with my wife, with no worry of infecting her with anything.

It might be wise for you to take a step back and examine your lifestyle.  I don't know how often you visit commercial sex workers, but twice in 6 months you have found it necessary to seek reassurance on this forum despite essentially no risk of infection.  If you continue such practices, you should of course continue to use condoms and otherwise practice safe sex, as you have been doing.  But it shouldn't be necessary to seek reassurance after every exposure.  If you continue to feel that need, despite having safe sex, I would advise you to stop such activities.  As Harry Truman supposedly said, "If you can't stand the heat, get out of the kitchen."

Zero risk from this sort of exposure.  Even entirely unprotected sex is quite inefficient in hepatitis B virus transmission and few escorts are infected; many have been immunized against it.  In any case, all persons should be immunized against HBV, regardless of their sexual lifestyles.  If you have not been vaccinated, discuss it with your doctor.

Thanks doctor, my big concern was the urine exposure and the risk i always hear about hep B, but this is a small chance? Thanks again
