Hsv

Is it possible to have an initial ob which is just the nerve pain from hsv without lesions.
I've had soreness in back, legs, hips, side of abdomen and some slight pains in pelvis.
No lesions yet that I've seen. 4.5 weeks post exposure. Pain started 2 weeks ago and has been persistent but has moved to mentioned parts of body somewhat intermittently. Also a feeling of being run down.
I have been negative hsv 1 and 2.  Igg testing done in london  Was tested 2 mos ago which way after any potential exp but before latest one.

I've read a lot about this neuralgia and now I'm worried as it seems to coincide. Have you seen an initial ob present this way? Is there cause to be concerned by this? Is this the prodrome before a pending initial ob.

No, this doesn't change anything.  Even very prominent lymph node enlargement would not change my opinion or advice.  It is herpetic skin lesions that cause the lymph node inflammation; i.e. herpes never causes lymph node inflammation without overt herpes lesions of the skin.

That's all for this thread.  I will have no further comments or advice.

Thank you dr. That is reassuring. Last question  What about the distinct tenderness in the inguinal lymph nodes. I did feel a small bump after touching around in that area. It's been quite sore.
Does this change any of your thoughts on this?
Would swollen lymphs be very pronounced?  Would a ct scan show abnormal sizing?

Great forum and your thoughts appreciated.

As for the exposures, condom protected vaginal sex is low risk for herpes, and hand-genital contact carries no measureble risk at all.

Why do I "feel strongly" about the symptoms you describe?  Because herpes imply doesn't behave that way.  Timing:  Initial herpes symptoms generally start within 5 days, sometimes 10 days -- and although 2.5 weeks is possible, it is rare.  Nature of symptoms:  initial symptoms almost always are overt skin lesions of herpes, not the aches and pains you describe.  Various symptoms like that are often listed as going along with herpes, but often without the key point:  herpes LESIONS may itch, burn, tingle etc -- or these symptoms may precede lesions, but only by a day or two.  In 30+ years in the STD business, I have never seen a patient with symptoms like yours who turned out to have herpes.

Try to let it go, and look to other causes of your symptoms.  My guess is it's nothing more than anxiety, magnifying normal body sensations.  But really, herpes is not an issue here.  There is no point in further testing for it.

Thank you dr for the response. The exposure was 4.5 weeks ago and it was sex with 3 different csw in czech repub. 1 protected sex the other 2 were me masturbating them and then immediately masturbating myself but no sex.
had been tested a year before exposure -negative.(first post should've read 12 mos not 2 mos) Tested again 8 days after exposure which realize too early but was concerned. It was neg for both 1 and 2 also which just confirmed was neg before this exposure. Symptoms started 2.5 weeks after exposure and remain today but will spread around a bit.  Example- Today I have tenderness in groin area by lymph nodes but a few days ago I had the soreness and tingly feeling in legs. A week ago it was a dull achy pain in the back.
Do not know if they have hsv or anything else.

Why do you feel strongly symptoms do not suggest herpes and only would be for recurrent?  Is it because the initial will show lesions due to lack of antibody?
Is there a risk of rubbing a lesion and then right away masturbating myself all with a lot of friction and for a large duration?
Is it still possible these symptoms are leading to a more classic ob with lesions?

Cheers.

Welcome to the forum.  I'll try to help.  Bottom line:  these symptoms are not due to herpes.

Herpes has highly variable clinical manifestations, so when anyone asks "is symptom X possible", the answer often is yes.  However, possible isn't the same as probable, and "nerve pain" is rarely if ever the only symptom of initial herpes.  Neuralgia, or other neuroopathic symptoms -- i.e. tingling, pain, and other symptoms attributable to nerve inflammation or irritation -- are pretty much limited to recurrent herpes, not the initial infection.  And when herpes is associated with such symptoms, typically are much more localized than you describe.  The widespread nature of your symptoms argues strongly against herpes.

If I understand correctly, you had a negative HSV blood test before the exposure you are concerned about and well before onset of these symptoms.  Please confirm that I understand the timing correctly.  Can you also say why that test was done?  Also it would be helpful if you could describe the exposure 4.5 weeks ago, and whether that persons is known to have HSV.

So based on the symptoms you describe, I very much doubt you have herpes.  But fill in the missing details and let's see if that change my judgment.

Regards--  HHH, MD
