definitely ngu

Dr Hook, I am sorry to write again but I am afraid I definitely have ngu and doctors here do not give precise answers. I am having profuse white discharge (for sure I am not imagining it) and as I told you before I tested negative for chlamydia and gonorrhea. My guess is I got cured with doxy and since my GF only took azythromycin she did not get cured and she reinfected me (we usually have protected sex but these past weekend we had unprotected sex). Questions are:
1)what type of bacteria/organism can cause ngu? (I have tested negaitve for trich, and herpes 1 and 2 also)
2) which of these are responsive to doxy but not to zithro?
3) how harmful are these?
4) I need my girlfriend to get cured, which antibiotics must she take? for how long? She probably has been infected for at least 6 months so I wonder if the course must be longer...
5) I think i caught ngu from my encounter outside my relationship (the condom must have broke). yet I wonder if there is any way that my gf passed it on to me? Is it possible for normal vaginal bacteria to cause ngu? Or did I definitely infect her?
5) should I take doxy again? or what should I take? for how long?
6) my Gf has been having abdominal pain, she went to her gynecologist and she said it was nothing, but now that I am sure we are sharing an infection I wonder if this pain is pid? From answers in this forum I understood non chlamydial ngu did no cause pid but I am scared anyway...

Please keep in my that my symptoms are not imaginary and that I am having strong and white discharge. This is not anxiety, also that local urologists are very familiar with the tradiitonal stds but not with ngu.

I have nothing more to say - we disagree and without a positve test or evidence that this is not something else, I will continue to agree with the doctor who has examined your GF.  .  EWH

Dr, I value and respect your opinion. Yet my reasoning tells me that my Gf is infecting me since for the past two months I have been feeling ok and having protected sex and the past weekend I had unprotected sex twice and today I am having discharge again. The only difference between the treatment that my girlfriend received and mine is that I took doxy for a week, she only took zithro (1gr 1 dose). I have been to two different urologists and both have said that I have non specific urethritis (here this means that I have inflammation and that they dont know why). Today I am having more discharge than I have ever had and much whiter. To summarize I am answerless and worried...

I do not thing your symptoms are imaginary however, if you both were treated without having sex in the interval between when you were both treated, the likelihood that your symptoms are due to NGU are miniscule.  I think you should be looking for other causes.   Your guilt and anxiety however do not seem to allow you to do this.  I will briefly answer your questions.

There is a long list of suggested organisms,.  You have been tested for the ones which are best described. The only other organism suggested to cause NGU is Mycoplasma genitalium and this is most often treated with azithromycin. In the best studies about 40% of cases of NGU have no demonstrable cause.  

2) which of these are responsive to doxy but not to zithro?
None.  If anything azithromycin would be thought to be most effective, as said above.

3) how harmful are these?
You know the answer to this. There is no good evidence that NGU which is negative for microorganisms is harmful to the person with NGU or his partners.

The starting point is for her to be tested, not to guess what is going on.

You assume you have NGU, I do not.

I think you need to see a urologist t to look for other causes.

The gynecologist's assessment supports my suspicion that this is not an STD.  Only you seem to think it is.

Sorry if I seem abrupt.  I continue to think you are over reacting and do not have an STD from your condom protected encounter.  Your test results and the information from your partner’s doctor seem to support this.  I think you need to look for another cause. EWH.
