hiv/syphilis/herpes/chancoid

Dear Dr Handsfiled;

I went to Tijuana Mexico about 5 weeks ago. I had sex with csw and the condom broke. She only work there for 3 days and said she was HIV negative.  She agreed and went to get tested for HIV 5 days after the condom broke. The result was negative and I went back to see the result. She since stop working and we maintain regular contact on Facebook. I went to the clinic a week after to get tested for Gono and Chlamydia, I was given Doxycycline for a week. Do I need to be concerned about HIV in this case? I am vaccinated for hep B awhile back.

About 10 days ago I went to Tj again and had protected sex with another csw, the condom did not break. Two days ago I was showering and I noticed a small slightly painful bump about 3/4 of an inch from the base of the penis on the pubic area. This is the area that is not cover by the condom. Over the last couple of day it seems more noticeable and slight more painful? I kept checking it. I did not noticed the lymph nodes are enlarge in the groin area.  I did not notice papules or chancre. Do this sound like syphilis? For syphilis do it always start from papues and then proceed to chancre?  And how long does it take from papues to chancre, one day?  one week?

I am also concern about chancroid and herpes! Does this sound like either one? I am planing to go the the clinic tomorrow and have it check out.

I am planning to get test tomorrow which is 7 weeks and 1 day after the condom break incident. I didn't know how long the condom was broken for because I was drink a lot. She got tested 5 days after our sexual encounter and she is HIV negative and I seen and have a copy of her result. Shes only in the business for 3 days and I believed her because I was with her on her first day also and it was protected sex.  Dr HHH seems to suggest 7 week test is conclusive but you seems to suggest testing out to 8 weeks. In this situation does it really matter? You told not to worry about it after her test came back negative but it's still lingering on my mind so that right I am getting tested. Thanks for your answer Dr!!!

I went to the std clinic and have one the doc look at it. She said is ingrown hair and folliculitis and she also said is not herpes, syphilis or anything to be concern about. She did told urine sample since I had a new partner. I should know the result next week. Wish me luck!

Since the csw also gave me oral sex do I need to be really concern about this? This is  a one time thing only.

My sense is that you are over reacting. No matter how you slice it, you need to be seen by a knowledgeable clinician who can guide assessment of this bump.  There are no STDs whihc appear a day, or even two days following exposure. EWH

CORRECTION TO THE LAST POST.

I have another questions. I am getting paranoid and now I remember this but I didn't think much of it until I was talking to my friend.  I had sex with another girl on Sunday night before I noticed the bump on Monday when I showered around noon. It was pretty likely that the bump were already there on SUNDAY night. We had condom protected sex but the bump is in the area that was not covered by the condom. If she has hiv and if this turns out to be herpes would that dramatically increase the risk of contracting hiv if some of her vaginal fluid come in contact with the bump. There was no open sore and there is still no open sore as I am writing this. Its just looks like some white thing in the small center around 1mm in diameter. I am very concern because the bump was not covered by the condom and during sex, it likely some vaginal fluid may get into that area. I showered right after the sex. When I shower around noon the next day that's when I feel around and noticed the tiny bump. Please let me know. This girl seems pretty careful insisting we use protection once things got hot and heavy.

This really does not sound like HSV (or chancroid).  As I said, have someone take a look tomorrow but do not squeeze it between now and then. EWH

Now it looks like there is some white stuff on the center but not 100% sure. The center is like 1mm very small. I bet if i squeeze it the white stuff will come out. Hopefully is just a pimple or folliculitis.

Your thoughts on all this?

How does herpes starts? Starting from blisters or starting from a bump? And the progression time?

I first noticed it by feeling it. It didn't look like anything. And two days later it doesn't look like a blister either its more noticeable. It is just slight red because I touch it so often. Does herpes start with a blister? Or does it start with a bump and becomes a blister? How long?

I hope and I am quite confident that I did not get HIV from her because she tested negative afterward and she and I was not sick one month after the episode. I am referring to ARS. This makes it very unlikely.

What about chancroid? I read it also progress rapidly like in one day.

What about Herpes? What are the symptoms and progression time?

There was no hair on the bump itself but there are a couple of hairs that is very closed to it. I had kind of similar bumps like this before like over a year and 9 months ago and the doc told me was folliculitis.They tested me for herpes but it was negative(culture).  But I am not sure if this is the same as before. And before I never had sex csw.

Welcome to our Forum. I'll be happy to comment and provide impressions however please have a health care professional take a look at the bump you describe. As I explain below, I doubt that it is an STD but to be sure you should get examined.

Sorry to hear of your condom failure. Condoms break about 1% of the time they are used.  The fact that your partner was tested and negative for HIV makes is very, very unlikely that you could have gotten HIV from the exposure.  My advice would be not to worry about it further.  Being tested with negative results and then treated with doxycycline will assure you that you did not get any other STD from the exposure you describe.  

Your second encounter in TJ was safe sex with little risk for STD because you were condom protected.  

The bump you describe sounds like folliculitis.  It does not sound like syphilis which is typically painless and progresses over just a day or two from a bump to a chancre.  Folliculitis occurs when the pores in your skin get plugged.  This allows that normal bacteria that are present on just about every-one's skin to grow and cause what is essentially a pimple.  Some of these just appear as red bumps which may or may not be  tender while others go on to actually form pimples.  In either case, they can be accompanied by swelling of nearby lymph nodes (FYI, swollen lymph nodes are a totally non-specific indicator that your body is reacting to something.    In some instances, if you look closely you can see a hair coming out of, or nearby the bump- this too is typical of folliculitis).

Hope this helps. I anticipate your doctor will tell you the bump is folliculitis. Please do not squeeze it between now and the time you are seen- this will only confuse things. EWH
