I was in the shower, and dismantled a shaving razor, I then proceeded to cut several small incisions into my penis, the cuts bled and for some time when I urinated they made a quiet gurgling sound.  This was a few weeks ago.

The wounds have since healed but I have started feeling pain in the area that they were in before.  Is this psychological, or is there something physically wrong here?

It's hard to say about the physical harm... have you seen a doctor for the cuts?
And, to whether or not this is psychological... I would say that whenever there's self-mutilation... there's a usually a psychological reason.
Have you seen a therapist/ counselor for that?
