Partial Erection and Low Arousal

I'm a 22 year old male who is suffering from erection problems for the past 6-7 months. My sexual arousal has diminished to the extent that I don't get an erection naturally. When I masturbate, I can only sustain the erection till the first ejaculation. Thereafter, it becomes very difficult to maintain the erection. My sperm has been coagulated for quite some time whenever I ejaculate. From what I've felt, the connective muscle behind the scrotum hasn't been functioning very well.
I'm also suffering from a chronic fissure for the past two years which I've been controlling using the Diltiazem 2% in organogel based preparation. I've also been affected by GERD for quite some time (started 5 years back) and my stomach isn't stable (dull pain below the diaphragm, gas and weird noises are a part of the current state that I'm facing). If it means anything in particular, my nipples have been puffy and soft for the past few months (3-4), though I have a fit physique and maintain a regimen. Also, putting on muscle mass has been a big struggle for me for the past few years.
I'm physically active to the maximum extent possible (lean built, weight: 72.1 Kgs) and consciously maintaining a healthy diet. Ultrasound of the abdomen has been inconclusive (no abnormalities). Urine test, stool test, prostrate test have come out without any abnormalities. MRI of ingroino-scrotal region has come out fine. Chest CT is normal. Physical examination of the scrotum and penis do not indicate any infection, nodules or abnormalities EXCEPT for a varicocele that was detected in my left testicle. Hygiene levels are fine, if not excellent (I take a bath every 2-3 days, yet no itching in the groin or infection).
I wish to ask what medical treatment should I seek? Any particular specialist/doctor whom I should consult? What is the underlying cause? I'm worried if it can't be rectified or if it's too late to recover from any damage due to the underlying condition.
Could it be indicative of something serious (Cancer, E.D. or Impotency)?    
I live in New Delhi, India (if that helps the diagnosis).
Would be happy if anyone can help me out. I intend to visit the hospital soon.
