Why can I only have an orgasm and ejaculate with rough intercourse?

I am quite caring and do not like hurting people, but I just need the hard friction feeling and foreskin being pulled tight when having intercourse and if this does not happen I can not have an orgasm. Is this a medical problem could this be fixed?

Can you masturbate and ejaculate? Does that have to be "rough" too? What about oral sex?

If you can orgasm through other, more gentle ways outside of intercourse, I'd say it's not medical. Are you trying different positions to see if that helps?

It's not that unusual for men to need the friction and speed to orgasm, but I guess I'd wonder how rough you're talking about.

I Can you masturbate and ejaculate but it Does  have to be rough too. in a simple why the forskin has to be pulled very tied/taught and far back, so if that is done inside my partner it can hurt her as i am big and long and can hit the cervix.So we get verry clinicall and finish by hard and fast masturbation
So essentially, everything has to be rough/hard and fast with the foreskin pulled tight in order for you to orgasm.

I can't find a thing about this being a medical condition. There's lots of things about foreskin being too tight, but that's when the penis is flaccid, or the foreskin is too loose, and it causes problems when the penis is flaccid.

I'm hoping a man will help answer this, as I don't have a penis and can't fully understand the different feelings, but have you ever talked to your doctor about it?
