OK - a couple Q’s:
1st, how old are you? When young guys are reaching their sexual peak during teen years, it’s pretty normal to have frequent night-time erections, and these can be very hard or even painful. On the other hand, there is such a thing as Priapism, which is a very hard erection that lasts way too long & won’t go away, even after orgasm & ejaculation. If that’s the case, it’s a pretty serious situation because repeated episodes can do permanent damage & eventually could result in permanent ED, so if this is what’s happening, you need to make that clear to your Dr.
2nd, how often do you masturbate or have sex? It’s possible if you’re not ejaculating frequently enough, you could experience some discomfort in your testicles & an uncomfortable feeling of fullness.
3rd, it’s normal to have nocturnal emissions (wet dreams), especially if you’re not getting orgasms & ejaculation by ‘normal’ means (masturbation or sex).
So I think in order to help you with this issue we need to know your age, how long these episodes last (if they could be Priapism), how often you masturbate or have sex, and whether you’ve had frequent nocturnal emissions. This would all be things I would think the Dr would ask you. If a reg Dr isn’t able to help with this, see a Urologist…

I'm having the same problem for almost a month ,need help how did you cure it?
Maybe your penis feels uncomfortable in the position it is in or the material used when you sleep.
You may need to ejaculate some more.
It is unusual symptom. You may consult urologist. or do further research in other websites till it is diagnosed.
