Hello.

What's probably happening is that you're hitting her cervix (the mouth of the uterus), which causes abdominal cramping. Short anatomy lesson: once a woman is sufficiently aroused, the uterus rises up, moving the cervix out of the way of a penis so it doesn’t get bumped. If you’re having penis-vagina sex (p-v) before she's sufficiently turned on, her cervix is vulnerable to bumping and irritation, which causes cramping and pain.

Please DO NOT ever have any kind of insertive sex unless she's very turned on. Sometimes, people rush toward what they consider “the main event” without realizing that a) sex should not have a goal; all touch can be pleasurable, and b) any kind of penetration can be painful if you’re not sufficiently turned on.

If she's not aroused, it’s probably because she's not getting enough clitoral stimulation during p-v. A major difference between women and men is that generally, the clitoris needs constant direct or indirect stimulation, unlike the penis. For most women, orgasm results from a constant circular motion around the shaft and glans (or head) of the clitoris. Also important is that once a woman’s orgasm begins, if the stimulation is removed, the orgasm will end. In contrast, once men have that first orgasmic contraction, not even a neutron bomb will stop their orgasm!

While many women enjoy p-v sex, for at least 40-50% of them, it usually doesn’t result in orgasm. Why? Because most p-v sex doesn’t provide the steady pressure and reliable stimulation women need for orgasm. During p-v sex, most men use an “in-out” motion that feels great for them, instead of the circular grinding motion that will stimulate the clitoris. So how about trading off once in awhile? Her first; then you—or vice-versa?

Let's talk about deep thrusting. Are you sure she likes it? There has to be communication in order for any relationship to work for both people, so it’s time to sit down and talk about what you both enjoy. Best of luck to you. Dr. J
