Try a Flip-Turn Sinus Flush, which uses gravity to get to infections in the upper sinuses, rather than fighting gravity like other saline rinses have to do. If it doesn't work, then you can eliminate sinus infection as a cause of your problem, which is some progress

I am a fellow sinusitis sufferer, about the same age. I have still not found a complete cure but I can share some of my research and personal experience.
- the key to curing sinus issues according to Ayurveda is to develop strong digestive fire. Some tips - consume raw ginger slices a few times a day, ingest top quality probiotics and prebiotics, cleanse digestive track now and again through fiber rich foods, enemas, etc.
- serrapeptase helps, if your digestion is weak buy a delayed release formula
- load up on magnesium and potassium. Inflammation is not compatible with ample magnesium stores in tissues. Most people are deficient. Buy bulk magnesium and potassium citrate powders, and make your own electrolyte drinks twice a day. Cell salts or liquid minerals are also good.
- dissolve biofilm with nasal sprays like xlear (xylitol is a key component) or sinupulse irrigator that pumps a solution of water, saline packets, Povidone Iodine Prep Solution and/or xylitol or baking soda.
- stay away from drafty windows, especially overnight while sleeping. Open window just a little and ensure drafts are not passing over your body.
- break sweat somehow every day, e.g. by exercising 20 minutes a day on a stationary bike, running, etc.
Try these things. To rely on a doctor to solve your issue is unrealistic. Everything is interconnected, so it's a good idea to view your health holistically.
