Try this exercise everyday and it will help with sleep. If you can do this for 2 weeks  then I will suggest another exercise to reduce weight.
Anulom Vilom - Deep Breath-in through left nostril keeping right nostril closed
then - Breath-out through right nostril keeping left nostril closed
then -Deep Breath-in through right nostril keeping left nostril closed
then - Breath-out through left nostril keeping right nostril closed
and repeat this cycle for upto 30  minutes twice a day(maximum 60 min/day).

MEDICAL PROFESSIONAL
Hi.

The one thing that is going to help you immensely is comming back to shape. Undergo a systematic weight reduction program. It will help a lot.

Ringing in the ear could be because of some other reason. Check with your ENT surgeon or neurologist.

All the best.

Your symptoms sound very similar to mine and I do have sleep apnea.  Left untreated, it can lead to heart problems, daytime drowsiness, memory issues, and many other health problems.  I would definitely consult a sleep specialist.  They can do a study to see if this is the problem.

The grinding teeth issue for me is related to TMJ.  Your dentist might be able to fit you with a nightguard which will prevent the tooth grinding.  I know if I don't wear my nightguard, I wake with horrible headaches.

Your symptons sound like sleep apnea or some form of breathing obstruction during sleep--like scarlet37 noted as well.

Have you asked a Dr about getting a sleep study done?  Overnight sleep study in a hospital?  They are usually very good about finding out if you have a sleep disorder--especially apnea.  The problems breathing, the grinding teeth, the jerking (because your body is reacting to lack of oxygen from the apnea),  the tired unrested feeling---they are classic sleep apnea symptons.  If you doctor can't figure the out, get a new doctor.

While I dont have sleep apnea, I have problems sleeping--falling asleep, staying asleep.  On average I get 3-4 hours sleep though I try for 8 hrs min.  I know the frustration of not feeling rested, achy, etc.  Again, please consider a sleep study.  Then if its not apnea, maybe consider the anxiety angle.  I think its more likely you have a physical problem affecting your sleep, which in turn, leads to increased anxiety--viscious cycle.   Good luck--let us know if you find some resolution.

Anxiety can cause symptoms such as feeling tired and achey, racing heart, a sense of gloom and doom, tightness in chest as well as many others. Having said that i must add that teeth grinding during sleep and severe snoring are symptoms of sleep apnea. Anxiety can be related to sleep disorders. If you have not done so already go to a sleeep specialist or a sleep ctr. that can properly diagnose you if indeed  do have a sleep issue.
