Hemi parasis

My wife-age 70 years-education 7th standard- had stroke in December 2009. Her left leg was affected and she could not walk. After six months of medicine and physiotherapy she can walk with the help of wheel chair or walker or cane. But simultaneously we observed in her short term memory loss ,personality change and psychological disturbances. She is very much afraid of falling and therefore does not want to attempt self walking. Some falls during self walking made her more adamant for self walking. Most of the time she likes to be in lying down position and does not want to sit.
Thus physically she is practically cured but mentally she is still patient. Her psychological state or changed personality is described below. I would like to have advice on this particular aspect after her stroke.

 1 Learning any thing new is very difficult as she forgets easily. She has to be told again and again. For many days same thing is to be repeated again and again. After long time she may learn. Temporary memory is affected and therefore sequence is very difficult to remember. She says same thing very often again and again. She does not remember if medicine is taken. She does not remember if lunch is taken. She does not remember what were the items in the lunch. She even forgets in which room she is. She very frequently goes for urination and forgets. After coming from urination again immediately asks for urination. Practically every half an hour she asks for urination and goes for. 
 2 She shouts loudly for what ever she wants. She is very persistent in getting what she wants. She talks loudly non-sense words and some times filthy words too. 
 3 She needs some body to be with her constantly and all the time. She will not get up and will not walk if some body is not around. This is because she is very much afraid of falling. Out of fear of falling she is not attempting to walk on her own. That fear is enhanced by seven minor falls she had.
 4 She experiences hallucinations. In the toilet some time she sees ghosts. She sees two snakes winding  around each other.
 5 Some time she becomes too emotional. Bouts of emotions.She weeps and laughs too simultaneously changing quickly from one mood to another. Some times even she slaps after shouting. Crying,scolding anybody without fear rightly or wrongly. She asks for sex frequently. Some times behaves like very understanding person and immediately next moment she will act and speak irrationaly.  
  6 Very sensitive to temperature changes. Makes ac and fan very often on and off. Asks to switch off lights. Asks to pull curtains to cut off light. Asks to switch off TV and Radio. Does not like noise. Does not like many peoples talking simultaneously. Does not like any body discussing any issues before her. Even if some body is talking on telephone she does not like.    
 7 She likes lying down in the bed all the time. She will not like to sit even for some time.
She has very poor stamina.
 8 Does not like rice and chapati. Likes hotel food and dishes. Taste is disturbed. She likes oily and sweet things.
 9 She does not like to be independent and want others to serve her. Very lazy woman.
Please advise me on this neuron-psychological treatment after stroke.

Another problem my wife is facing is severe itching. Back of neck,hand elbows,portion of legs between knee and foot are the areas itching severely. She scrape so much that some time it bleeds.We have tried lotion for scrabies but did not help. Given treatment for allergy but did not help.Thought of soaps detergents but did not help.Even treatment for ecsima did not help much.Is it normal that patient gets itching as after effect of stroke.It is also told that while cureing itching will be there.If any body can clear the things from own experience or expert can help in this matter.Itching is frequent and severe and embarasing many times.

My wife can walk now 50 meters unaided. Thre are all signs that she will fully recover her walking ability. Her behavior is still abnormal. She utters nonsense words some time. But she is improving on this side too. She is now motivated. Within three months she should be normal.

I'm 72 years old I've got a stroke 18 years ago after that I've got parasis leftside also have depression. please give some Ideas how to treat. I was and am always under massage and medication.

This was an extremely good reply by petitep in another stroke forum. I think it applies here.
He has to be clear of trying to do ANY multi-tasking when he's listening. He most likely cannot multi-task anymore with a R-stroke. Let me share how simple or hard this can be... When you are talking to him... If he is not looking directly at you, paying specific attention to what you are saying, he will not retain it, this includes a glance to an earring, to the tv in the background, noise in the room it has to be silent, a thought that comes into his mind while you are talking, picking up a glass. ALL of those things while he is trying to listen or communicate is out of the question. The INSTANT he does ANY of the following while trying to listen to you, he will not hear you speaking, thus not being able to answer your question or retain the information long enough to hear the weather.

Speech practices with him with 1 - 3 - 5 words at a time, depending upon his level, (Repeat after me... "Apple, Grass, Flower" IF he does not know one of those three words for instance they then ASK you, "what else were you paying attention to at that time" For me it was glancing over to a window, to see a calendar behind where she was sitting or touching the eraser of the pencil. Then they continue to practice so that you don't get side-tracked with ANYTHING else while trying to listen.
He will most likely at this point not be able to retain the information long enough to be useful, find out where his current level is.. at that point with me, I couldn't retain information long enough from the beginning of the sentence to the end in order to figure out how to respond.
Books are great once he gets to that level. If you get auditory anything it needs to be SIMPLE language, very SHORT sentences at a time, when you ask him questions it will take a while for him to process sentences. it will be easier for him to master everyday language first, then work up to a paragraph.
The speech therapist has already identified what he struggles with and where his strengths are and techniques you can practice with him... Ask him or her for details.
The race is on... He only has 6 months to have a HUGE improvement from where he was at the time of the stroke for 6 months out... the 6 months to a year, there is significantly less improvement... after one year you've come to a plateau in the eyes of your medical team. You are still improving after that but it is a LOT less, you will only see improvements if you compare from 1 month ago instead of daily or weekly improvements.

I compare her before and after stroke. Even before stroke she was lazy that is how she got stroke. But question is why she should be lazy even before stroke? Her left side carotid artery is completely blocked. This was detected only after stroke.
As a result of stroke I find her loss is very small compared to all other patients. She even can walk but for fear of fall. Her speechnis perfectly normal. Infact we observed that her hearing and seeing power has increased. She has got sufficient power to shout very loudly. She is physically not much handicapped but as a after effect of stroke we find her temporary memory loss. Old things she recollects vividly. I understand that she deserves love but some time really so unberable and one looses control. She is so moody in one stroke she praises and blames me.
I am worried about her behavior as a result of memory loss. Thank you for showing concern. Inspite of medical attention situation is not showing much improvement.

Sounds like you are in serious denial that your wife had a major stroke.  

I cannot believe you called her a "Very lazy woman."  Get over it, and go with the flow.  Moreover, if you can, make sure she gets occupational, speech and physical therapy while it's still early.  My mother's hubby deprived my Mom of this and we had not say so b/c they were married.  Don't "scold" her because she does not have the ability to think cognitively 100%  Good grief!
